腹腔镜下侧腹壁悬吊术及其改良术式治疗盆腔器官脱垂的研究进展

摘要 盆腔器官脱垂是好发于中老年女性的一类疾病,手术往往是消除脱垂症状的最终选择,然而该病的治疗术式目前尚未达成统一的标准,现行术式均有其明显优缺点和适用范围。近年来伴随着女性对术后美观和保留子宫的需求增加,腹腔镜下侧腹壁悬吊术作为一种在保留子宫和切除子宫术式中均有良好治疗效果的技术得到了临床医师的青睐,其学习曲线短、风险小、出血少且并发症发生率低,该术式及其改良术式在治疗前、中腔室脱垂中展现出的高治愈率,体现了该术式的普适性和实用性,有望成为腹腔镜下骶骨固定术的替代术式。而在不同类型的网片材料中,聚丙烯合成网片展现出了更低的并发症发生率,是腹腔镜手术首选的网片修补材料。 Pelvic organ prolapse is a disease prevalent in middle-aged and elderly women.Surgery is often the ultimate choice to eliminate the symptoms of prolapse.However,there is currently no uniform standard for the treatment of this disease,and the existing surgical methods have obvious advantages,disadvantages,and applicability.In recent years,clinicians have favored laparoscopic lateral suspension with mesh as a technique for preserving the uterus and achieving postoperative beauty for women.This surgery has proven to have good therapeutic results in both uterus preservation and hysterectomy,with short learning curve,low risk,minimal bleeding and low complication rate.The high success rate of this surgery and its modifications in the treatment of anterior and intermediate ventricular prolapses reflects the universality of the procedure and the high rate of complications.The universality and practicality of this procedure is expected to become a substitute for laparoscopic sacralcolpopexy.Among the different types of mesh materials,polypropylene synthetic mesh exhibits a lower incidence of complications and is the preferred extracorporeal repair material for laparoscopic surgery.
