肺移植术后原发性移植物功能障碍机制的研究进展

摘要 肺移植是终末期肺病唯一有效的治疗手段。近年来手术技术、供肺保存以及移植后的管理策略有所进步,但是肺移植术后的患者生存率并没有明显改善,与其他实体器官移植相比仍是最低的。原发性移植物功能障碍(primary graft dysfunction,PGD)是肺移植术后早期急性肺损伤,多发生在术后的48~72 h内,术后发生率可达5%~80%[1-3],一般情况下可以在术后5~10 d改善,但是严重的PGD占移植后30 d死因的50%[4],并且与基线肺功能降低以及慢性同种异体肺移植物功能障碍(chronic lung allograft dysfunction,CLAD)风险相关[5]。本文总结PGD的病理生理机制,为其预防诊断治疗提供新思路。
