基于CCTA的冠周脂肪影像组学在鉴别急性冠脉综合征和稳定性冠心病中的价值 被引量:1

摘要 目的:探讨基于冠状动脉CT血管成像(coronary computed tomographic angiography,CCTA)的冠状动脉周围脂肪组织(pericoronary adipose tissue,PCAT)影像组学模型在鉴别急性冠状动脉综合征(acute coronary syndrome,ACS)和稳定性冠心病(stable coronary artery disease,SCAD)中的应用价值。方法:回顾性分析97例冠心病患者影像学资料及临床资料,分为ACS组48例和SCAD组49例。将狭窄冠状动脉周围脂肪组织设为感兴趣区(region of interest,ROI),提取出脂肪衰减指数(fat attenuation index,FAI)和脂肪影像组学特征。按照7∶3比例构建影像组学模型,97例患者随机分为训练组67例和测试组30例。通过单因素相关系数分析、单因素Logistic回归分析以及最小绝对收缩与选择算子(LASSO)回归进行影像组学特征筛选并构建预测模型。采用受试者工作特征(receiver operating characteristic,ROC)曲线分析两种模型的诊断效能。结果:从基于CCTA检查勾画的冠周脂肪ROI的图像中获取788个影像组学特征,最终筛选出6个特征来构建冠周脂肪影像组学预测模型,基于提取的脂肪衰减指数构建FAI模型。冠周脂肪影像组学预测模型在训练组及测试组中曲线下面积(area under the curve,AUC)(0.881、0.884)均高于FAI模型的AUC(0.721)。冠周脂肪影像组学预测模型在测试组的准确度、敏感度、特异度分别为80.0%、66.7%、93.3%。结论:基于CCTA检查的冠周脂肪影像组学模型在鉴别急性冠脉综合征和稳定性冠心病中明显优于FAI模型,具有更高的应用价值,有助于及时、准确发现高危患者,为临床采取干预措施提供依据。
