慢性牙周炎患者唾液中miR-107、miR-106a表达及其诊断价值

摘要 目的探究慢性牙周炎患者唾液中miR-107、miR-106a的表达及其诊断价值。方法选取2020年6月至2022年6月在该院诊治的110例慢性牙周炎患者作为观察组。根据牙周炎病情严重程度将110例慢性牙周炎患者分为轻度组(47例)、中度组(38例)、重度组(25例)。依据慢性牙周炎活动期的诊断标准将患者分为活动期组(58例)与静止期组(52例)。另选取同期在该院检查无口腔疾病的110例健康者作为对照组。收集所有研究对象的临床病历资料,比较观察组与对照组基线资料及唾液中miR-107、miR-106a表达水平,比较不同病情严重程度、不同活动期患者唾液中miR-107、miR-106a表达水平。绘制受试者工作特征(ROC)曲线分析miR-107、miR-106a单独及2项指标联合检测对慢性牙周炎的诊断价值。结果观察组与对照组体质量指数比较,差异无统计学意义(P>0.05);观察组牙龈出血、龈下菌斑比例均高于照组,差异均有统计学意义(P<0.05)。观察组唾液中miR-107、miR-106a表达水平均低于对照组,差异均有统计学意义(P<0.05)。轻度组、中度组和重度组唾液中miR-107、miR-106a表达水平比较,差异均有统计学意义(P<0.05);且轻度组miR-107、miR-106a表达水平均高于中度组和重度组,中度组miR-107、miR-106a表达水平均高于重度组,差异均有统计学意义(P<0.05)。与静止期组比较,活动期组唾液中miR-107、miR-106a表达水平降低,差异均有统计学意义(P<0.05)。ROC曲线分析结果显示,miR-107、miR-106a单独诊断慢性牙周炎的曲线下面积分别为0.771、0.742,均小于2项指标联合诊断的0.859(Z=2.200、2.867,P<0.05)。结论miR-107、miR-106a表达水平在慢性牙周炎患者中均降低,可作为慢性牙周炎诊断的分子标志物。 Objective To investigate the expression and diagnostic value of miR-107 and miR-106a in saliva of patients with chronic periodontitis.Methods A total of 110 patients with chronic periodontitis diagnosed and treated in the hospital from June 2020 to June 2022 were selected as the observation group.According to the severity of periodontitis,110 patients with chronic periodontitis were divided into mild group(47 cases),moderate group(38 cases)and severe group(25 cases).According to the diagnostic criteria of chronic periodontitis in active stage,the patients were divided into active stage group(58 cases)and static stage group(52 cases).At the same time,110 healthy people without oral diseases were selected as the control group.The baseline data and the expression levels of miR-107 and miR-106a in saliva of the observation group and the control group were compared,and the expression levels of miR-107 and miR-106a in saliva of patients with different disease severity and different active stages were compared.The receiver operating characteristic(ROC)curve was drawn to analyze the diagnostic value of miR-107,miR-106a alone and the combined detection of the two indicators for chronic periodontitis.Results There was no significant difference in body mass index between the observation group and the control group(P>0.05).The proportions of gingival bleeding and subgingival plaque in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The expression levels of miR-107 and miR-106a in the saliva in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).There were significant differences in the expression levels of miR-107 and miR-106a in saliva among the mild group,the moderate group and the severe group(P<0.05).The expression levels of miR-107 and miR-106a in the mild group were higher than those in the moderate group and the severe group,and the expression levels of miR-107 and miR-106a in the moderate group were higher than those in the severe group,and the differences were statistically significant(P<0.05).Compared with the static stage group,the expression levels of miR-107 and miR-106a in the saliva of the active stage group decreased,and the difference was statistically significant(P<0.05).ROC curve analysis showed that the area under the curve of miR-107 and miR-106a in the diagnosis of chronic periodontitis were 0.771 and 0.742 respectively,which were less than 0.859 of the combined diagnosis of the two indicators(Z=2.200,2.867,P<0.05).C onclusion The expression levels of miR-107 and miR-106a decrease in patients with chronic periodontitis,which can be used as molecular markers for the diagnosis of chronic periodontitis.
