摘要
目的观察身体质量指数(body mass index,BMI)对桡骨远端骨折严重程度和手术时间的影响。方法我科自2016年6月至2022年7月收治桡骨远端骨折患者84例,依照桡骨远端骨折的AO分型进行分组:A组29例(23-A2.2、23-A2.3、23-A3型),B组26例(23-B型),C组29例(23-C型)。记录患者的年龄,性别,吸烟史,受伤至医院时间,是否伴有糖尿病、高血压病史,受伤的侧别,手术时间及BMI。利用卡方检验、Logistic回归及线性回归分析的方法评估年龄、性别、吸烟史、受伤的侧别、受伤至医院时间、是否伴有糖尿病高血压病史、BMI与A,B,C三组之间的关系。使用线性回归模型分析BMI对桡骨远端骨折手术时间的影响,均以P<0.05为差异具有统计学意义。结果84例患者获得完整随访,使用卡方检验比较A、B、C三组的年龄、性别、吸烟史、受伤至医院时间、是否伴有糖尿病高血压病史、BMI和受伤侧别之间的差异,发现年龄、性别、吸烟史和BMI在组间差异有统计学意义(P<0.05)。对年龄、性别、吸烟史、BMI、受伤侧别、受伤至医院时间、是否伴有糖尿病高血压病史进行多因素Logistic回归分析,筛选出年龄和BMI是致桡骨远端骨折的危险因素(P<0.05)。把A组作为对照组进一步分析BMI对B组和C组的影响,得出BMI是发生23-B型和23-C型桡骨远端骨折的危险因素。使用线性回归分析BMI对桡骨远端骨折手术时间的影响,得出BMI与手术时间有显著相关性(P<0.01)。结论BMI是发生23-B型和23-C型桡骨远端骨折的危险因素,并且BMI能影响桡骨远端骨折患者的手术时间,随着BMI的增高,手术时间相应延长。
Objective To observe the effect of body mass index(BMI)on the severity and surgical time of distal radius fractures.Methods From June 2016 to July 2022,84 patients with distal radius fractures were treated in our department.They were divided into groups according to the AO classification of distal radius fractures:29 cases in Group A(23-A2.2,23-A2.3,23-A3),26 cases in Group B(23-B),and 29 cases in Group C(23-C).The patient′s age,gender,smoking history,time of injury to hospital,whether there is a history of diabetes,hypertension,injured side,operation time and BMI were recorded.The relationship between age,gender,smoking history,injured side,time of injury to hospital,diabetes and hypertension history,BMI and the three groups(Group A,B,C)were evaluated by Chi-square,Logistic regression and linear regression analysis.The impact of BMI on the surgical time of distal radius fractures was analyzed using a linear regression model,with a statistically significant difference of P<0.05.Results Eight-four patients were follow-up completely.Chi-square test was used to compare the differences among the age,gender,smoking history,time of injury to hospital,whether there was diabetes and hypertension history,BMI and injured side in Group A,B and C.It was found that there were significant differences between the age,gender,smoking history and BMI among the groups(P<0.05).Multivariate Logistic regression analysis was conducted on age,gender,smoking history,BMI,injured side,time of injury to hospital,and whether there was diabetes and hypertension history.Age and BMI were screened as risk factors for distal radius fractures(P<0.05).Group A was used as the control group,and further analysis was conducted on the impact of BMI on Group B and Group C.It was found that BMI was a risk factor for the occurrence of 23-B and 23-C distal radius fractures.Using linear regression analysis,it was found that BMI had a significant correlation with surgical time for distal radius fractures(P<0.01).Conclusion BMI is a risk factor for the occurrence of 23-B and 23-C distal radius fractures,and it can affect the surgical time of patients with distal radius fractures.As BMI increases,the surgical time is correspondingly prolonged.
