小切口辅助复位有限内固定联合支架外固定治疗胫骨远端粉碎性骨折的临床效果分析

Clinical Effect of Small Incision Assisted Reduction Limited Internal Fixation Combined with Stent External Fixation in Treatment of Comminuted Fracture of Distal Tibia

目的探究小切口辅助复位固定+支架外固定对于胫骨骨折患者的治疗效果。方法随机选取2019年3月-2022年3月福建省将乐县总医院收治的80例胫骨远端粉碎性骨折患者为研究对象,按照不同治疗方法将患者分为对照组和观察组,每组40例。对照组给予开放复位胫骨远端板内固定治疗;观察组胫骨远端内和/或外侧小切口辅助复位、有限内固定联合外固定架治疗,比较两组手术治疗效果及恢复情况。结果两组手术时长和失血量对比,差异无统计学意义(P均>0.05)。观察组住院及伤口愈合时间短于对照组,差异有统计学意义(P均<0.05)。观察组优良率为85.0%,优于对照组的62.5%,差异有统计学意义(χ^(2)=5.230,P<0.05)。观察组并发症发生率为5.0%,少于对照组的20.0%,差异有统计学意义(χ^(2)=4.114,P<0.05)。治疗后,观察组的生活质量优于对照组,差异有统计学意义(P<0.05)。结论应用小切口辅助复位有限内固定联合支架外固定治疗临床效果更佳,能缩短愈合和住院时间,并发症少,是一种良好的固定方法。

Objective To explore the therapeutic effect of small incision assisted reduction fixation plus stent external fixation on patients with tibial fractures.Methods A total of 80 patients with comminuted distal tibial fractures admitted to Jiangle County General Hospital of Fujian Province from March 2019 to March 2022 were selected randomly as the research objects.According to different treatment methods,the patients were divided into control group and observation group,with 40 cases in each group.The control group was treated with open reduction and internal fixation of distal tibial plate;the observation group was treated with distal tibial internal and/or lateral small incision assisted reduction,limited internal fixation combined with external fixator.The surgical treatment effect and recovery of the two groups were compared.Results There was no statistically significant difference in surgical duration and blood loss between the two groups(both P>0.05).The hospitalization and wound healing time in the observation group were shorter than those in the control group,and the differences were statistically significant(both P<0.05).The excellent rate of the observation group was 85.0%,which was better than the control group's 62.5%,and the difference was statistically significant(χ^(2)=5.230,P<0.05).The incidence of complications in the observation group was 5.0%,which was lower than the control group's 20.0%,and the difference was statistically significant(χ^(2)=4.114,P<0.05).After treatment,the quality of life in the observation group was better than that in the control group,and the difference was statistically significant(P<0.05).Conclusion The application of small incision assisted reduction and limited internal fixation combined with stent external fixation has better clinical effect,can shorten healing and hospitalization time,and has fewer complications,so it is a good fixation method.
