目标导向式健康威胁行为扭转延续护理结合生活希望计划护理模式在糖尿病肾病维持性血液透析中的应用价值

Application value of goal-oriented health-threatening behavior reversal continuous care combined with life hope planning nursing mode in maintenance hemodialysis of diabetic kidney disease

目的探讨目标导向式健康威胁行为扭转延续护理结合生活希望计划护理模式在糖尿病肾病维持性血液透析中的应用价值。方法选取2019年5月至2022年5月收治的120例糖尿病肾病维持性血液透析患者为研究对象,根据护理方式不同将其分为对照组和观察组,各60例。对照组实施常规护理,观察组在常规护理基础上实施目标导向式健康威胁行为扭转延续护理结合生活希望计划护理模式。比较两组的干预效果。结果干预后,观察组的疾病认知、健康信念及健康行为评分高于对照组(P<0.05)。干预后,观察组的Herth希望量表各维度评分高于对照组(P<0.05)。干预后,观察组的平均血糖(MBG)、最大血糖波动幅度(LAGE)、血肌酐(Scr)及血尿素氮(BUN)水平均低于对照组(P<0.05)。结论目标导向式健康威胁行为扭转延续护理结合生活希望计划护理模式可提高糖尿病肾病维持性血液透析患者知信行与希望水平,减轻血糖波动,改善肾功能,值得推广。

Objective To explore the application value of goal-oriented health-threatening behavior reversal continuous care combined with life hope planning nursing mode in maintenance hemodialysis of diabetic kidney disease.Methods A total of 120 diabetic kidney disease patients undergoing maintenance hemodialysis admitted from May 2019 to May 2022 were selected as the study objects.According to different nursing methods,the patients were divided into control group and observation group,with 60 cases in each group.The control group received routine nursing,and the observation group received goal-oriented health-threatening behavior reversal continuous care combined with life hope planning nursing mode on the basis of routine nursing.The intervention effects of the two groups were compared.Results After intervention,the scores of disease cognition,health belief and health behavior in the observation group were higher than those in the control group(P<0.05).After intervention,the scores of each dimension of Herth hope scale in the observation group were higher than those in the control group(P<0.05).After intervention,the levels of mean blood glucose(MBG),largest amplitude of glycemic excursion(LAGE),serum creatinine(Scr)and blood urea nitrogen(BUN)in the observation group were lower than those in the control group(P<0.05).Conclusion The goal-oriented health-threatening behavior reversal continuous care combined with life hope planning nursing mode can improve the level of knowledge-attitude-practice and hope of diabetic kidney disease patients undergoing maintenance hemodialysis,reduce blood glucose fluctuation,and improve renal function,which is worth promoting.
