HPV分型检测对宫颈细胞学阴性患者的临床价值

摘要 目的探讨人乳头瘤病毒(Hunan papillomavirus,HPV)分型在宫颈细胞学阴性患者中的临床价值。方法回顾选取我院2020年1月至2022年10月期间的106名细胞学阴性且HPV阳性的患者,均行阴道镜检查和宫颈活检,分析HPV不同型别与宫颈病变的关系。结果106例宫颈细胞学阴性患者中检出HPV分型前五位为HPV52型29例(27.36%)、HPV16型25例(23.58%)、HPV51型11例(10.38%)、HPV18型10(9.43%)、HPV56型9例(8.49%),HPV以单一感染为主,占70.75%。宫颈活检病理分级为宫颈炎症的86例81.13%,CINⅠ7例(6.60%),CINⅡ+12例(12.27%),未检出宫颈癌病例。12例CINⅡ+中感染的HPV分型以HPV16型8例(66.67%)和HPV52型6例(50%)为主。结论HPV分型检测对于临床分流有重要意义,宫颈细胞学阴性且HPV16型阳性,直接行阴道镜及宫颈活检检查。HPV52型和HPV58型可根据临床情况行阴道镜检查,其他高危型定期检查TCT、HPV分型和随访。 Objectives To investigate the clinical value of human papillomavirus(HPV)typing in patients with negative cervical cytology.Methods A retrospective analysis were conducted on 106 cytologically-negative and HPV-positive patients in our hospital from January 2020 to October 2022.All patients underwent colposcopy and cervical biopsy,and the relationship between different types of HPV and cervical lesions was analyzed.Results Among the 106 patients with negative cervical cytology,the top five HPV types were HPV 52(27.36%),HPV16(23.58%),HPV 51(10.38%),HPV 18(9.43%),and HPV 56(8.49%).Single HPV infection was the most common,accounting for 70.75%of the total sample.According to the pathological grading of cervical biopsy,there were 86 cases(81.13%),7(6.60%)cases,and 12(12.27%)cases diagnosed as cervical inflammation,CINII and CINⅡ+,respectively,and no cervical cancer was detected.Among the 12 cases of CINⅡ+,8 cases(66.67%)were HPV 16 and 6 cases(50%)were HPV 52.Conclusions HPV typing detection is of great importance for clinical shunting.For those with negative cervical cytology and HPV 16 positive,colposcopy and cervical biopsy can be performed directly.Those diagnosed as HPV 52 and HPV 58 positive can be examined by colposcopy according to clinical conditions,and other high-risk types can be examined regularly by TCT,HPV typing and follow-ups.
