轻度认知障碍患者脑灰质体积与认知及运动功能水平的关系研究

Relationship between gray matter volume and cognitive and motor function in patients with mild cognitive impairment

摘要 目的 探讨轻度认知障碍(mild cognitive impairment,MCI)患者脑灰质体积与认知及运动功能水平的关系。方法 纳入2020年1月至2022年12月于广东同江医院神经内科就诊MCI患者74例及同期年龄性别匹配体检健康人70例,收集并分析MCI患者与健康人一般资料、认知功能指标、运动功能指标及灰质体积;采用偏相关分析评估差异脑区灰质体积与认知功能及运动功能指标间相关性,采用线性回归模型分析全脑灰质体积与运动功能指标间关系。结果 偏相关分析结果显示,MCI患者感兴趣区域(region of interest,ROI)灰质体积与定向、记忆及执行功能间呈正相关(P <0.05),与日常生活能力量表评分呈负相关(P<0.05);ROI-2灰质体积与日常生活能力量表评分呈负相关(P<0.05);ROI-4灰质体积与计算功能呈正相关(P<0.05);ROI-5灰质体积与执行功能呈正相关(P<0.05);ROI-6灰质体积与简易精神状态检查量表评分、剑桥认知检查量表中文版评分总分及计算功能间呈正相关(P<0.05),与日常生活能力量表评分间呈负相关(P<0.05)。偏相关分析结果显示,MCI患者ROI-5灰质体积与步速间呈正相关(P<0.05)。线性回归模型分析结果显示,MCI患者灰质体积左侧枕上回与起立行走测试时长呈负相关(P<0.05);灰质体积双侧额叶直回、右眶部额中回、右距状裂/枕上回及左中央前回均与步速呈正相关(P<0.05)。结论 MCI患者颞叶内侧体积缩小可能与认知功能损伤有关;同时运动功能还受枕叶、前额叶及中央前回等灰质体积影响。 Objective To investigate the relationship between gray matter volume and cognitive and motor function in patients with mild cognitive impairment(MCI).Methods 74 patients with mild cognitive impairment and 70 healthy people by age and sex matched were chosen in the period from January 2020 to December 2022 in our hospital.The general data,cognitive function indexes,motor function indexes and gray matter volume were analyzed.Partial correlation analysis was used to evaluate the correlation between gray matter volume in different brain regions and cognitive function and motor function indexes.Linear regression model was used to analyze the relationship between gray matter volume and motor function indexes.Results Partial correlation analysis showed that ROI-1 gray matter volume in MCI patients was positively correlated with orientation,memory and executive function,and negatively correlated with daily living ability scale scores(P<0.05).There was a negative correlation between ROI-2 gray matter volume and daily living ability scale score in MCI patients(P<0.05).There was a positive correlation between ROI-4 gray matter volume and computational function in MCI patients(P<0.05).There was a positive correlation between ROI-5 gray matter volume and executive function in MCI patients(P<0.05).ROI-6 gray matter volume in MCI patients was positively correlated with the score of the Simple Mental State Scale,the total score of the Chinese version of the Cambridge Cognitive Scale and computing function(P<0.05),and negatively correlated with the score of the daily living Ability Scale(P<0.05).Conclusions The reduced volume of the medial temporal lobe in MCI patients may be related to the impairment of cognitive function.Motor function was also affected by the volume of gray matter in occipital,prefrontal and anterior central gyrus.
