目标调整在前列腺癌幸存者照顾者的负担与心理困扰中的中介作用

The mediating role of goal adjustment in the burden and psychological distress in caregivers of prostate cancer survivors

目的通过研究目标调整在前列腺癌幸存者照顾者的负担与心理困扰之间的潜在调节作用,探讨有效干预策略。方法选取2019年1月至2021年2月广州市第一人民医院诊治的139例前列腺癌幸存者及其照顾者作为研究对象。采用目标调整量表(AGS)、Zarit护理负担量表(ZBI)、简式简明心境问卷(POMS-SF)评估前列腺癌幸存者照顾者,并进行相关性分析和结构方程模型分析。结果AGS-目标脱离(GD)、AGS-目标重置(RS)、ZBI、POMS-SF评分分别为(3.60±1.18)、(3.13±1.25)、(34.76±8.12)、(40.48±5.68)分。ZBI调节POMS-SF路径中,AGS-GD(χ^(2)=35.568,P<0.05)及AGS-RS(χ^(2)=24.688,P<0.05)部分中介模型、完全中介模型间的模型拟合指数差异具有统计学意义,其调节效应均属于部分中介效应。结论目标脱离能力和目标重置能力在前列腺癌幸存者照顾者的负担及心理困扰调节机制中起着重要作用。因此,通过目标调整的干预策略可以降低照顾者的负担和心理困扰。

Objective To explore effective intervention strategies by studying the potential moderating effect of goal adjustment in the burden and psychological distress in caregivers of prostate cancer survivors.Methods A total of 139 prostate cancer survivors admitted to Guangzhou First People′s Hospital from January 2019 to February 2021 and their caregivers were selected as the study objects.Achievement goals scale(AGS),Zarit burden interview(ZBI),and short form of the profile of mood states(POMS-SF)were used to evaluate the caregivers of prostate cancer survivors,and correlation analysis and structural equation model analysis were performed.Results The scores of AGS-goal disengagement(GD),AGS-re-engagement scale(RS),ZBI,and POMS-SF scales were(3.60±1.18),(3.13±1.25),(34.76±8.12),and(40.48±5.68)points,respectively.In the POMS-SF pathway regulated by ZBI,the model fitting index of AGS-GD(χ^(2)=35.568,P<0.05)and AGS-RS(χ^(2)=24.688,P<0.05)was different between partial mediation models and complete mediation models,with statistically significance,and their regulatory effects were all partial mediation effect.Conclusions The abilities to detach and reset goals play an important role in the burden and psychological distress regulation mechanisms in caregivers of prostate cancer survivors.Therefore,intervention strategies based on goal adjustment can reduce caregivers′burden and psychological distress.
