血清sICAM-1水平与OEM伴不孕患者IVF-ET后早期流产的相关性分析

Analysis of the relationship between serum soluble intercellular adhesion molecule-1 level and early abortion after in vitro fertilization-embryo transfer in patients with ovarian endometriosis and infertility

目的分析血清可溶性细胞间黏附分子-1(sICAM-1)水平与卵巢型子宫内膜异位症(OEM)伴不孕患者体外受精-胚胎移植(IVF-ET)后早期流产的相关性。方法选取2020年1月至2022年12月于海口玛丽医院接受IVF-ET且成功妊娠的90例OEM伴不孕患者作为研究对象,根据妊娠后早期流产发生情况分为流产组(n=17)与未流产组(n=73)。比较两组基线资料及实验室指标,重点分析血清sICAM-1与OEM伴不孕患者IVF-ET后早期流产的关系。结果与未流产组相比,流产组移植日内膜厚度较小,移植后第12~14天血清人绒毛膜促性腺激素(β-hCG)水平较低,血清sICAM-1水平较高(P<0.05);Logistic回归分析显示,移植日内膜厚度、移植后第12~14天血清β-hCG和sICAM-1水平均是OEM伴不孕患者IVF-ET后早期流产的影响因素(P<0.05);通过受试者工作特征(ROC)曲线发现,血清sICAM-1对OEM伴不孕患者IVF-ET后早期流产具有一定预测价值。结论血清sICAM-1水平过高提示OEM伴不孕患者IVF-ET后早期流产发生风险较高,可作为预测患者早期流产发生的辅助指标。

Objective To analyze the relationship between serum soluble intercellular adhesion molecule-1(sICAM-1)level and early abortion after in vitro fertilization-embryo transfer(IVF-ET)in patients with ovarian endometriosis(OEM)and infertility.Methods A total of 90 patients with OEM and infertility who received IVF-ET and had successful pregnancy in Haikou Mary Hospital from January 2020 to December 2022 were selected as the study subjects.They were divided into abortion group(n=17)and non-abortion group(n=73)according to the occurrence of early abortion after pregnancy.Baseline data and laboratory indicators were compared between the two groups,with emphasis on analyzing the relationship between serum sICAM-1 and early abortion after IVF-ET in patients with OEM and infertility.Results Compared with non-abortion group,the endometrial thickness on the day of transplantation in the abortion group was smaller,and the serum level of human chorionic gonadotropin-β(β-hCG)was lower on the 12-14 days after transplantation,while the serum level of sICAM-1 was higher(P<0.05).Logistic regression analysis showed that endometrial thickness on the day of transplantation,serumβ-hCG and sICAM-1 levels on the 12-14 days after transplantation were the influencing factors of early abortion after IVF-ET in OEM patients with infertility(P<0.05).The receiver operating characteristic(ROC)curve showed that serum sICAM-1 had a certain predictive value for early abortion after IVF-ET in OEM patients with infertility.Conclusions The high level of sICAM-1 in serum indicates a higher risk of early abortion after IVF-ET in patients with OEM and infertility,and can be used as an auxiliary indicator for prediction of early abortion in patients.
