系统性红斑狼疮患者骨密度与部分免疫指标的相关性研究

Correlation between bone mineral density and some immune indexes among patients with systemic lupus erythematosus

目的:探讨系统性红斑狼疮(SLE)患者骨密度(BMD)与部分免疫指标的相关性及免疫指标是否影响SLE患者BMD。方法:采用回顾性研究方法,选取2023年1月—5月在广州市番禺区中心医院因患SLE住院的成年患者43例为研究对象。采用双能骨密度仪检测患者腰椎骨密度,期间监测ESR、补体C3、补体C4、IgA、IgG、IgM、CRP等免疫指标,并记录患者年龄、性别、体重、身高等个人基本资料。结果:43例SLE患者的平均年龄为(51.12±14.13)岁,有67.44%患者存在骨代谢异常,有51.16%出现骨量减少,16.28%存在骨质疏松。通过Pearson方法分析观察指标之间的直线关系,多元回归分析显示补体C3、IgM及年龄是BMD的主要影响因素。结论:免疫指标浓度下降将会导致系统性红斑狼疮患者骨密度改变,随着年龄增大和Ig M及补体C3指标浓度下降将会导致患者骨密度显著下降。

Objective To investigate the correlation between bone mineral density(BMD)and some immune indexes in patients with systemic lupus erythematosus(SLE),and whether immune indexes affect BMD in patients with SLE.Methods Using a retrospective study method,43 adult patients admitted to the Panyu Central Hospital,Guangzhou from January to May 2023 due to SLE disease were selected as the study subjects.Lumbar vertebra bone density was detected by dual-energy bone densitometer,during which immune indexes such as ESR,complement C3,complement C4,IgA,IgG,IgM and CRP were monitored,and basic personal data such as age,sex,weight and height were recorded.Results The average age of 43 SLE patients was(51.12±14.13)years old.67.44%of patients had abnormal bone metabolism,51.16%had decreased bone mass,and 16.28%had osteoporosis.The linear relationship between the observed indicators was analyzed by Pearson method.Multiple regression analysis showed that complement C3,IgM and age were the main influencing factors of BMD.Conclusion The decrease of immune index concentration will lead to the change of bone mineral density in patients with systemic lupus erythematosus,and the increase of age and the decrease of IgM and C3 index concentration will lead to the significant decrease of bone mineral density in patients with SLE.
