摘要
目的 探讨X线摄影的乳腺影像报告和数据对乳腺肿块性质的鉴别价值。方法 选取2022年1月~2023年1月就诊于我院的接受X线摄影检查且确诊伴有乳腺肿块的患者80例。分析所有患者的X线摄影下的病理结果、X线表现结果、钙化形态、钙化分布、肿块形态以及肿块分布,比较X线摄影的乳腺影像报告和数据系统结果与病理结果。结果 80例患者中共检测出97个肿块,术后病理确诊67个为良性,30个为恶性,良性肿块形态规则、边缘平整光滑、密度和腺体相同,或略高于腺体;恶性肿块可能伴有钙化,形态不规则,边缘不光滑伴有毛刺状,密度显著高于腺体。基于X线摄影的乳腺影像报告和数据系统结果与病理结果的灵敏度、特异度、准确率、阳性预测值、阴性预测值分别为73.33%、108.33%、89.69%、91.67%、89.04%。X线摄影的乳腺影像报告和数据系统结果与病理结果的Kappa值为0.775。结论 基于X线摄影的乳腺影像报告和数据系统对乳腺肿块性质的鉴定有较高研究价值。
Objective:To explore the discriminatory value of radiography-based breast image reporting and data systems in properties of breast masses.Methods:A total of 80 patients confirmed with breast masses by radiography in the hospi⁃tal were retrospectively analyzed between January 2022 and January 2023.The pathological results and findings of X-ray,calcification morphology,calcification distribution,mass morphology,and mass distribution in all patients were analyzed.The diagnostic results of breast masses by radiography-based breast image reporting and data system and pathological ex⁃amination were compared.Results:There were 97 masses detected in 80 patients,and 67 were diagnosed as benign and 30 as malignant by postoperative pathology.In benign masses,there was regular morphology,smooth edges,and the same or slightly higher density than glands.In malignant masses,there may be calcification,irregular morphology,unsmooth edges with spicule signs,and significantly higher density than glands.The sensitivity,specificity,accuracy,positive predic⁃tive value,and negative predictive value of the radiography-based breast image reporting and data system results with patho⁃logical findings were 73.33%,108.33%,89.69%,91.67%,and 89.04%,respectively.The Kappa value of the radiographybased breast image reporting and data system results with pathological findings was 0.775.Conclusion:Radiographybased breastimaging reports and data systems are ofhigh research value for the identification ofthe nature ofbreast masses.
