腹膜前间隙补片修补对切口疝大鼠碱性成纤维生长因子(bFGF)的影响

Influence of preperitoneal space mesh repair on basic fibroblast growth factor(bFGF)in rats with incisional hernia

摘要 目的观察大鼠切口疝腹膜前间隙补片修补术后不同时间腹膜及周围组织中碱性成纤维生长因子(bFGF)的变化趋势,分析植入补片对术后修复作用的影响。方法选用48只SD大鼠,随机分为正常组(n=8)、切口疝模型组(n=8)、有疝补片修补术后1、4、8、12周组(n=8)。将聚丙烯补片植入有疝补片修补组大鼠的腹膜前间隙。正常组取正常腹膜及周围组织,切口疝模型组及有疝补片修补组取切口旁腹膜及周围组织进行RT-PCR检测,获得各组大鼠腹膜及周围组织中bFGF mRNA基因相对表达量并进行比较。结果与正常组比较,切口疝模型组、有疝补片修补术后1周组腹膜及周围组织中bFGF mRNA表达明显升高(P<0.01);其余各组与正常组比较差异无统计学意义(P>0.05)。有疝补片修补术后1、4周组与切口疝模型组比较,差异无统计学意义(P>0.05),术后8、12周组腹膜及周围组织中bFGF mRNA表达较切口疝模型组显著降低(P<0.05)。有疝补片修补组各时相比较,与术后1周组比较,术后8周、12周组腹膜及周围组织中bFGF mRNA表达明显降低(P<0.05);其余各组之间比较差异无统计学意义(P>0.05)。结论在行切口疝腹膜前间隙补片修补术时,手术创伤可能是刺激腹膜及其周围组织产生bFGF的主要因素,此反应在术后1周最明显。聚丙烯补片理论上可以作为bFGF的载体,让其缓慢释放,但本研究并未提示有明显的作用,这可能需要后续进一步研究确认。 Objective To observe the trend of basic fibroblast growth factor(bFGF)in the peritoneum and surrounding tissues at different times after preperitoneal space mesh repair surgery in rats with incisional hernia,and to analyze the effect of the implanted mesh on postoperative repair.Methods Forty-eight SD rats were randomly divided into a normal group(n=8),an incisional hernia model group(n=8),and groups at 1,4,8,and 12 weeks post-hernia preperitoneal space mesh repair surgery(n=8 each).A polypropylene mesh was implanted in the preperitoneal space of the rats in the hernia mesh repair groups.The normal group's regular peritoneum and surrounding tissues,and the incisional hernia model and hernia mesh repair groups'peritoneum and surrounding tissues near the incision were taken for RT-PCR to obtain the relative expression levels of bFGF mRNA.Results Compared with the normal group,the expression of bFGF mRNA in the peritoneum and surrounding tissues significantly increased in the incisional hernia model group and the 1-week post-hernia mesh repair group(P<0.01).The differences were not significant compared to the normal group for the remaining groups(P>0.05).Compared with the incisional hernia model group,there was no significant difference in the 1 and 4 weeks post-repair groups(P>0.05),while the expression of bFGF mRNA in the peritoneum and surrounding tissues decreased at 8 and 12 weeks post-repair,with a significant difference(P<0.05).Comparing different time points within the hernia mesh repair group,the expression of bFGF mRNA decreased at 8 and 12 weeks post-repair compared to the 1-week post-repair group,with a significant difference(P<0.05);there was no significant difference among the other groups(P>0.05).Conclusion During incisional hernia mesh repair of the preperitoneal space,surgical trauma may be the main factor that stimulates the production of bFGF in the peritoneum and its surrounding tissues,and this reaction is most obvious in the first week after surgery.Theoretically,polypropylene meshes can be used as carrier of bFGF for slow release,but this study did not suggest that there is an obvious effect,which may need to be confirmed by further research.
