健脾益心方加减联合盐酸胺碘酮治疗老年慢性心律失常患者的效果

Effects of Jianpi Yixin formula combined with Amiodarone hydrochloride in treatment of elderly patients with chronic arrhythmia

目的:观察健脾益心方加减联合盐酸胺碘酮治疗老年慢性心律失常患者的效果。方法:选取2022年1月至2023年1月该院收治的80例老年慢性心律失常患者进行前瞻性研究,按抛掷硬币法将其分为对照组和观察组各40例。对照组行盐酸胺碘酮治疗,观察组在对照组基础上联合健脾益心方加减治疗,比较两组临床疗效,治疗前后中医证候积分、心功能指标[每搏输出量(SV)、左心室射血分数(LVEF)]水平、生化指标[C反应蛋白(CRP)、同型半胱氨酸(Hcy)、利尿肽(BNP)]水平,以及治疗期间不良反应发生率。结果:观察组治疗总有效率为90.00%(36/40),高于对照组的72.50%(29/40),差异有统计学意义(P<0.05);治疗后,观察组心悸怔忡、肢倦乏力、失眠多梦、腹胀便溏等中医证候积分均低于对照组,差异有统计学意义(P<0.05);治疗后,观察组SV、LVEF水平均高于对照组,差异有统计学意义(P<0.05);治疗后,观察组CRP、Hcy、BNP水平均低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:健脾益心方加减联合盐酸胺碘酮治疗老年慢性心律失常患者可提高治疗总有效率和心功能指标水平,降低中医证候积分和生化指标水平,效果优于单纯盐酸胺碘酮治疗。

Objective:To observe effects of Jianpi Yixin formula combined with Amiodarone hydrochloride in treatment of elderly patients with chronic arrhythmia.Methods:A prospective study was conducted on 80 elderly patients with chronic arrhythmia admitted to the hospital from January 2022 to January 2023.According to the coin tossing method,they were divided into control group and observation group,40 cases in each group.The control group was treated with Amiodarone hydrochloride tablets,while the observation group was treated with Jianpi Yixin formula on the basis of that of the control group.The clinical efficacy,the TCM syndrome scores,the cardiac function indexes[stroke volume(SV),left ventricular ejection fraction(LVEF)]levels,the biochemical indexes[C-reactive protein(CRP),homocysteine(Hcy),diuretic peptide(BNP)]levels before and after the treatment,and the incidence of adverse reactions during the treatment were compared between the two groups.Results:The total effective rate of treatment in the observation group was 90.00%(36/40),which was higher than 72.50%(29/40)in the control group,and the difference was statistically significant(P<0.05).After the treatment,the scores of TCM syndromes such as palpitation,limb fatigue,insomnia and dreaminess,abdominal distension and loose stools in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After the treatment,the levels of SV and LVEF in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).After the treatment,the levels of CRP,Hcy and BNP in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Jianpi Yixin formula combined with Amiodarone hydrochloride in the treatment of the elderly patients with chronic arrhythmia can improve the total treatment efficiency and the cardiac function index levels,and reduce the TCM syndrome scores and the biochemical index levels.Moreover,it is superior to single Amiodarone hydrochloride tablets.
