盐酸纳美芬辅助治疗重型颅脑损伤患者的效果及对其神经功能的影响

Effect of Nalmefene Hydrochloride in Adjuvant Treatment of Patients withSevere Craniocerebral Injury and Its Effect on Neurological Function

摘要 目的:探讨盐酸纳美芬辅助治疗重型颅脑损伤患者的效果及对其神经功能的影响。方法:将本院2020年6月至2022年12月期间96例重型颅脑损伤患者按随机数字表法分为对照组(n=48)和观察组(n=48);对照组给予盐酸纳洛酮治疗,观察组给予盐酸纳美芬治疗;选择临床疗效、血清神经损伤相关因子水平、神经功能及不良反应为观察指标,对两组患者治疗效果进行比较。结果:观察组治疗总有效率高于对照组(P<0.05);治疗1周,观察组患者血清神经元特异性烯醇化酶(NSE)、β-内咖肽、S100B水平均较对照组低(P<0.05);治疗1周、治疗2周,观察组患者的脑电双频指数(BIS)值较对照组高(P<0.05);两组不良反应发生率差异无统计学意义(P>0.05)。结论:盐酸纳美芬辅助治疗重型颅脑损伤患者可以提高治疗总有效率,降低血清神经损伤相关因子水平,改善患者的神经功能,不良反应发生率无明显增加。 Objective:To investigate the efficacy of nalmefene hydrochloride as an adjunctive treatment for patients with severe traumatic brain injury and its impact on their neurological function.Methods:A total of 96 patients with severe traumatic brain injury admitted to our hospital between June 2020 and December 2022 were randomly divided into a control group(n=48)and an observation group(n=48).The control group received naloxone hydrochloride treatment,while the observation group received nalmefene hydrochloride treatment.The clinical efficacy,serum levels of neurotoxic factors,neurological function,and adverse reactions were selected as the observation indicators to compare the therapeutic effects of the two groups.Results:The overall effective rate of the observation group was higher than that of the control group(P<0.05).One week after treatment,serum levels of neuron-specific enolase(NSE),beta-endorphin,and S100B were lower in the observation group compared to the control group(P<0.05).The bispectral index(BIS)values were higher in the observation group than in the control group at 1-and 2-week follow-up(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Nalmefene hydrochloride as an adjunctive treatment for patients with severe traumatic brain injury can improve the overall efficacy of treatment,reduce serum levels of neurotoxic factors,improve neurological function,and have no significant increase in adverse reaction rate.
