瘢痕子宫再次妊娠二次剖宫产与非瘢痕子宫初次剖宫产产妇的临床研究

Clinical study on the second cesarean section of scarred uterus and the first cesarean section of non scarred uterus inparturients

目的研究瘢痕子宫再次妊娠二次剖宫产与非瘢痕子宫初次剖宫产产妇的临床情况。方法选取本院50例无剖宫产史的非瘢痕子宫产妇为对照组,50例有部宫产史的瘢痕子宫产妇为试验组,均行剖宫产手术,比较两组的手术情况、不良妊娠结局、并发症发生情况、新生儿出生情况。结果试验组手术时间、术后恶露持续时间长于对照组,术中出血量、术后出血量多于对照组,差异有统计学意义(P<0.05);试验组不良妊娠结局发生率、新生儿出生后室息率、术后并发症发生率高于对照组,新生儿阿普加评分低于对照组(P<0.05)。结论相较于非瘢痕子宫初次剖宫产产妇,瘢痕子宫产妇二次剖宫产手术操作时间更长,手术难度更大,出血量更多,不良术后并发症发生率较高。

Objective To study the clinical situation of pregnant women undergoing a second cesarean section in a scarred uterus and a first cesarean section in a non scarred uterus.Methods 50 non scarred uterine parturients without a history of cesarean section were selected as the control group,and 50 scarred uterine parturients with a history of cesarean section were selected as the experimental group.Cesarean section surgery was performed,and the surgical situation,adverse pregnancy outcomes,incidence of complications,and newborn birth status of the two groups were compared.Results The experimental group had longer surgical time and longer postoperative lochia than the control group,with more intraoperative and postoperative bleeding than the control group,and the difference was statistically significant(P<0.05);The incidence of adverse pregnancy outcomes,neonatal asphyxia,and postoperative complications in the experimental group were higher than those in the control group,and the Apgar score of newborns was lower than that in the control group(P<0.05).Conclusion Compared to first-time cesarean section women with non scarred uterus,second cesarean section women with scarred uterus have longer operation time,greater surgical difficulty,more bleeding,and a higher incidence of adverse postoperative complications.
