动态增强磁共振成像定量参数与乳腺癌患者术后复发转移及临床病理特征的相关性研究

Correlation between quantitative parameters of DCE-MRI and postoperative recurrence,metastasis and clinicopathological characteristics of breast cancer patients

目的探讨动态增强磁共振成像(DCE-MRI)定量参数与乳腺癌患者术后复发转移及临床病理特征的相关性。方法选取2018年1月至2021年12月在濮阳市人民医院接受乳腺癌改良根治术的120例乳腺癌患者作为研究对象。收集患者临床病历资料。采用DCE-MRI对乳腺癌患者肿瘤组织进行扫描检测。分析DCE-MRI的放射组学特征参数乳腺癌症患者术后患者临床病理特征的相关性。结果K^(trans)和K_(ep)数值在不同肿瘤大小、组织学分级、淋巴结转移、ER表达和PR表达之间存在统计学差异(P<0.05);除组织学分级外,不同肿瘤大小、淋巴结转移、ER表达和PR表达之间的V_(e)数值比较则无统计学差异(P<0.05)。发生复发转移的患者K^(trans)和K_(ep)数值均要高于未发生复发转移的患者(P<0.05),而两组间Ve数值比较则无统计学差异(P>0.05)。肿瘤大小、组织学分级、淋巴结转移、ER表达和PR表达与K^(trans)和K_(ep)具有相关性(P<0.05),而V_(e)与组织学分级具有相关性(P<0.05),与肿瘤大小、淋巴结转移、ER表达和PR表达无相关性(P>0.05)。结论DCE-MRI成像参数K^(trans)、K_(ep)和V_(e)与乳腺癌患者临床病理特征和术后复发转移存在相关性。通过DCE-MRI检测能够有助于对乳腺癌患者病情和术后转移复发进行评估。

Objective To explore the correlation between quantitative parameters of dynamic contrast-enhanced mag⁃netic resonance imaging(DCE-MRI)and postoperative recurrence,metastasis and clinicopathological characteristics of breast cancer patients.Methods A total of 120 patients with breast cancer who received modified radical mastectomy in our hospital from Jan 2018 to Dec 2021 were selected as research subjects.Clinical medical records of patients were col⁃lected.The tumor tissue of breast cancer patients was scanned by DCE-MRI.The correlation between postoperative clinicopathological features of patients with breast cancer and radiomic features of DCE-MRI was analyzed.Results The values of Ktrans and Kep were significantly different in tumor size,histological grade,lymph node metasta⁃sis,ER expression and PR expression(P<0.05).There were no significant differences in Ve values between different tumor sizes,lymph node metastasis,ER expression and PR expression except histological grade(P<0.05).Ktrans and Kep values in patients with recurrence and metastasis were higher than those in patients without recurrence and metastasis(P<0.05),but there was no significant difference in Ve values between the two groups(P>0.05).Tumor size,his⁃tological grade,lymph node metastasis,ER expression and PR expression were correlated with Ktrans and Kep(P<0.05),while Ve was correlated with histological grade(P<0.05),but not with tumor size,lymph node metasta⁃sis,ER expression and PR expression(P>0.05).Conclusion DCE-MRI imaging parameters K^(trans),K_(ep)and V_(e)are correlated with clinicopathological features and postoperative recurrence and metastasis of breast cancer patients.DCE-MRI can help to evaluate the condition and postoperative metastasis and recurrence of breast cancer patients.
