子宫内膜息肉宫腔镜术后复发的影响因素及预防措施

Investigation of influencing factors and preventive interventions in patients with endometrial polyp after hysteroscopic surgery

目的探究子宫内膜息肉患者宫腔镜手术后复发的影响因素,针对性分析预防干预措施。方法回顾性分析2020年3月至2022年7月乐平市中医医院收治的92例子宫内膜息肉患者的临床资料,均行宫腔镜手术,根据术后复发情况分为复发组和未复发组,并收集患者年龄、产次、体质量指数、病程、流产次数、息肉大小、肿瘤坏死因子-α(TNF-α)及白细胞介素-6(IL-6)阳性情况等资料,采用单因素、多因素分析影响子宫内膜息肉患者宫腔镜手术后复发的相关因素。结果92例子宫内膜息肉患者中,13例宫腔镜手术后复发,复发率为14.13%(13/92);单因素分析显示,两组产次、病程、息肉大小比较,差异无统计学意义(P>0.05);两组年龄、体质量指数、流产次数、TNF-α及IL-6阳性情况比较,差异有统计学意义(P<0.05);Logistic回归分析显示:年龄≥35岁、体质量指数≥28 kg/m^(2)、流产次数≥3次、TNF-α及IL-6阳性是子宫内膜息肉患者宫腔镜手术后复发的高危因素(P<0.05)。结论子宫内膜息肉患者宫腔镜手术后存在一定复发风险,与年龄、体质量指数、流产次数、TNF-α及IL-6阳性均相关,临床需提高重视度,采取有效预防干预措施,以降低复发风险。

Objective To explore the factors affecting the recurrence of patients with endometrial polyps after hysteroscopic surgery,and to analyze the preventive interventions.Methods Clinical data of 92 patients with endometrial polyps treated in Leping Hospital of Traditional Chinese Medicine from March 2020 to July 2022 were retrospectively analyzed.All patients underwent hysteroscopic surgery and were divided into recurrence group and non-recurrence group according to postoperative recurrence.Data such as age,birth time,body mass index,course of disease,number of abortions,polyp size,tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)positivity were collected.Univariate and multivariate analysis was used to analyze the related factors affecting the recurrence of patients with endometrial polyps after hysteroscopic surgery.Results Among 92 patients with endometrial polyps,13 cases recurred after hysteroscopic surgery,and the recurrence rate was 14.13%(13/92).Univariate analysis showed that there was no significant difference in birth time,disease course and polyp size between the two groups(P>0.05).There were significant differences in age,body mass index,abortion frequency,TNF-αand IL-6 positivity between the two groups(P<0.05).Logistic regression analysis showed that age≥35 years old,body mass index≥28 kg/m^(2),number of abortions≥3 times,TNF-αand IL-6 positive were high risk factors for recurrence of patients with endometrial polyps after hysteroscopic surgery(P<0.05).Conclusion There is a certain risk of recurrence in patients with endometrial polyps after hysteroscopic surgery,which is related to age,body mass index,number of abortions,TNF-αand IL-6 positivity.Clinical attention should be paid and effective preventive interventions should be taken to reduce the risk of recurrence.
