家庭为中心下的产科护理对初产妇分娩恐惧及分娩结局的影响

The Effect of Family-Centered Obstetric Care on Fear of Childbirth and Childbirth Outcomes of Primipara

目的:探讨家庭为中心下的产科护理对初产妇分娩恐惧及分娩结局的影响。方法:以苏州市立医院自2020年1月1日~2023年1月31日抽取的102例初产妇作为研究对象,按照随机分组法分为对照组和观察组各51例,对照组采用常规产科护理,观察组在常规护理基础上实施家庭为中心下的产科护理;比较两组干预前后分娩恐惧程度[采用分娩恐惧量表(CAQ)]、分娩自我效能[采用简化中文版分娩自我效能量表(CBSEI-32)],分娩结局。结果:干预后,观察组CAQ中胎儿健康、产痛危害、自我控制、医疗护理评分及总分均低于对照组(P<0.05);干预后,观察组CBSEI-32中结果期望、分娩自我效能期望评分及总分均高于对照组(P<0.05);观察组分娩结局优于对照组(P<0.05)。结论:采取家庭为中心下的产科护理可有效改善初产妇的分娩恐惧,降低剖宫产率,同时改善分娩体验,提升分娩结局,利于初产妇顺利度过分娩期。

Objective:To explore the effect of family-centered obstetric care on fear of childbirth and childbirth outcomes of primipara.Method:A total of 102 primipara selected from Suzhou Municipal Hospital from January 1,2020 to January 31,2023 were selected as the research subjects and randomly divided into control group and observation group,with 51 cases in each group.The control group received routine obstetric care,while the observation group received family-centered obstetric care on the basis of routine care.Childbirth fear score using the Childbirth Attitude Questionnaire(CAQ),the self-efficacy of childbirth using the Simplified Chinese version of the Childbirth Self-Efficacy Scale(CBSEI-32),and the delivery outcomes were compared between the two groups.Results:After intervention,the CAQ scores of fetal health,labor pain hazards,self-control,medical care,and total score in the observation group were lower than those in the control group(P<0.05).After intervention,the CBSEI-32 scores of outcome expectation,delivery self-efficacy expectation score,and total score in the observation group were higher than those in the control group(P<0.05).The delivery outcome of the observation group was better than that of the control group(P<0.05).Conclusion:Family-centered obstetric care can effectively improve the fear of childbirth in primipara,reduce the cesarean section rate,improve the delivery experience,enhance delivery outcomes,and facilitate the smooth passage of the childbirth period.
