PKRBT术后联合不同膀胱灌注化疗方案对浅表性膀胱癌患者的疗效及对其sICAM-1等指标和预后的影响评估

Assessment of the Efficacy of Postoperative PKRBT Combined with Different Bladder Perfusion Chemotherapy Regimens in Patients with Superficial Bladder Cancer and the Impact on Their sICAM-1 and Other Indicators and Prognosis

目的探讨PKRBT术后联合不同膀胱灌注化疗方案对浅表性膀胱癌患者的疗效及对其sICAM-1等指标和预后的影响。方法选取113例浅表性膀胱癌患者,均行PKRBT术,术后根据膀胱灌注化疗方案差异分为2组。研究组患者75例采用吡柔比星联合吉西他滨膀胱灌洗治疗,而对照组患者38例则采用吡柔比星膀胱灌洗治疗,比较2组患者的临床疗效等差异。结果研究组的临床总有效率(77.3%)高于对照组(44.7%)(P<0.05);研究组术后治疗后不良反应发生率(5.3%)显著低于对照组(31.6%)(P<0.05)。治疗后2组的血清指标均较术前降低,且研究组下降更甚(P<0.05)。术后1年,研究组的复发率(2.7%)显著低于对照组(23.7%)(P<0.05)。结论PKRBT术后给予吡柔比星联合吉西他滨膀胱灌洗治疗浅表性膀胱癌可显著提高疗效,同时可改善患者预后,值得临床推广应用。

Objective To investigate the efficacy of postoperative PKRBT combined with different bladder perfusion chemotherapy regimens in patients with superficial bladder cancer and the impact on their sICAM-1 and other indicators and prognosis.Methods 113 patients with superficial bladder cancer were selected,all of whom underwent PKRBT and were divided into 2 groups according to the difference in bladder irrigation chemotherapy regimen after surgery.Among them,75 patients in the study group were treated with piroxicam combined with gemcitabine bladder irrigation,while 38 patients in the control group were treated with piroxicam bladder irrigation to compare the clinical efficacy and other differences between the 2 groups.Results The total clinical effective rate of the study group(77.3%)was higher than that of the control group(44.7%)(P<0.05);The incidence rate of postoperative post-treatment adverse reactions of the study group(5.3%)was significantly lower than that of the control group(31.6%)(P<0.05);After the treatment,the serum indexes of the 2 groups were lower than those before the operation and the more lower in the study group(P<0.05);1 year after the operation,the recurrence rate of the study group(2.7%)was significantly lower than that of the control group(23.7%)(P<0.05).Conclusion The administration of piroxicam combined with gemcitabine bladder irrigation after PKRBT can significantly improve the efficacy of patients with superficial bladder cancer,and at the same time improve the prognosis of patients,which is worthy of clinical promotion and application.
