宫颈癌腹腔镜根治术的疗效及手术部位感染的影响因素分析

The Effect of Laparoscopic Radical Resection of Cervical Cancer and Influencing Factors of Surgical Site Infection

目的探讨宫颈癌患者经腹腔镜治疗后手术部位发生感染的影响因素以及治疗有效率。方法收集78例行腹腔镜下宫颈癌根治患者的一般临床资料,统计临床治疗有效率、手术部位感染发生率、将患者分成手术部位感染组和未感染组,酶联免疫吸附法检测患者血清中白细胞介素(IL)-6、IL-1β和肿瘤坏死因子α(TNF-α)水平,分析患者术后手术部位发生感染的影响因素。结果宫颈癌腹腔镜根治术的总有效率为93.59%;手术部位感染发生率为17.95%(P<0.05);体质量指数、手术持续时间、引流管留置时间、糖尿病以及IL-6、IL-1β和TNF-α水平等因素与手术部位发生感染具有关联性(P<0.05);经Logistic回归分析,体质量指数、手术持续时间、引流管留置时间是手术部位发生感染的危险因素。结论宫颈癌腹腔镜根治术具有良好疗效,但是术后手术部位感染风险较高,临床上可根据本研究列出的术后感染影响因素制定干预措施,从而有效降低术后手术部位感染的风险。

Objective To investigate the influencing factors of surgical site infection in patients with cervical cancer after laparoscopic treatment and the effective rate of treatment.Methods General clinical data of 78 patients undergoing laparoscopic radical cervical cancer treatment were collected,clinical treatment effectiveness and incidence of surgical site infection were analyzed,and patients were divided into surgical site infection group and uninfected group.Serum levels of interleukin(IL-6),IL-1β and tumor necrosis factorα(TNF-α)were detected by enzym-linked immunosorbent assay.The influencing factors of postoperative surgical site infection were analyzed.Results The total effective rate of laparoscopic radical resection of cervical cancer was 93.59%.The incidence of surgical site infection was 17.95%(P<0.05).Body mass index(BMI),duration of surgery,retention time of drainage tube,diabetes mellitus,IL-6,IL-1β and TNF-α levels were associated with infection of surgical site(P<0.05).Logistic regression analysis showed that body mass index,operation duration and drainage tube indwelling time were the risk factors for surgical site infection.Conclusion Laparoscopic radical resection of cervical cancer has good curative effect,but the risk of postoperative surgical site infection is high.Clinically,intervention measures can be formulated according to the influencing factors of postoperative infection listed in this study,so as to effectively reduce the risk of postoperative surgical site infection.
