早期分阶段肺康复训练在极重度慢性阻塞性肺疾病急性加重住院患者中的应用价值

Application Value of Early Stage Pulmonary Rehabilitation Training in Hospitalized Patients with Acute Exacerbation of Extremely Severe Chronic Obstructive Pulmonary Disease

目的:分析早期分阶段肺康复(RP)训练在极重度慢性阻塞性肺疾病急性加重(AECOPD)住院患者中的应用价值。方法:选取2021年3月—2023年2月贺州市人民医院收治的极重度AECOPD住院患者96例为研究对象,采用随机数字表法分为对照组与观察组,各48例。对照组实施常规康复指导,观察组在对照组基础上实施早期分阶段RP训练。比较两组运动耐力指标、改良版呼吸困难量表、圣乔治呼吸问卷和医院焦虑抑郁量表评分。结果:干预后,两组峰值摄氧量、无氧阈、峰值代谢当量、峰值氧脉搏、峰值负荷功率、6min步行距离水平高于干预前,且观察组高于对照组,差异有统计学意义(P<0.05);干预后,两组呼吸困难指数、圣乔治呼吸问卷、焦虑抑郁量表评分低于干预前,且观察组低于对照组,差异有统计学意义(P<0.05)。结论:早期分阶段RP训练应用于极重度AECOPD患者中,能有效缓解患者的呼吸道症状,提高运动耐力及生活质量,改善负性情绪。

Objective:To analyze the application value of early stage pulmonary rehabilitation(RP)training in patients with acute exacerbation of extremely severe chronic obstructive pulmonary disease(AECOPD).Methods:A total of 96 patients with extremely severe AECOPD were selected as the study objects at Hezhou People's Hospital from March 2021 to February 2023,and were divided into control group and observation group by random number table method,with 48 cases in each group.The control group received routine rehabilitation guidance,and the observation group received early stage RP training based on the control group.The exercise endurance index,the modified dyspnea scale,the St.George's Breathing Questionnaire and the Hospital Anxiety and Depression Scale were compared between the two groups.Results:After intervention,the levels of peak oxygen uptake,anaerobic threshold,peak metabolic equivalent,peak oxygen pulse,peak load power and 6 min walking distance in 2 groups were higher than before intervention,and the observation group was higher than the control group,the difference was statistically significant(P<0.05);after intervention,the dyspnea index,St.George's breathing questionnaire and anxiety and depression scale scores of the two groups were lower than before intervention,and the observation group was lower than the control group,the difference was statistically significant(P<0.05).Conclusion:Early stage RP training can effectively relieve respiratory symptoms,improve exercise endurance and quality of life,and improve negative emotions in patients with extremely severe AECOPD.
