脑胶质瘤术后发生下肢深静脉血栓的影响因素分析

Analysis of the influencing factor of lower extremity deep venous thrombosis after glioma surgery

目的探讨脑胶质瘤患者术后发生下肢深静脉血栓的影响因素,为临床防治提供参考和依据。方法将76例脑胶质瘤患者根据术后是否发生下肢深静脉血栓分为下肢深静脉血栓组(n=21)与非下肢深静脉血栓组(n=55)。比较两组患者的临床资料,采用多因素Logistic回归分析筛选脑胶质瘤患者术后发生下肢深静脉血栓的危险因素。结果下肢深静脉血栓组中年龄﹥60岁及合并高血压、脑血栓、下肢静脉曲张的患者比例均高于非下肢深静脉血栓组,差异均有统计学意义(P﹤0.05)。多因素Logistic回归分析结果显示,年龄﹥60岁及合并高血压、脑血栓、下肢静脉曲张均为脑胶质瘤患者术后发生下肢深静脉血栓的独立危险因素(P﹤0.01)。结论年龄﹥60岁及合并高血压、脑血栓、下肢静脉曲张均为脑胶质瘤患者术后发生下肢深静脉血栓的独立危险因素,可针对上述因素制订相关临床措施,预防脑胶质瘤患者术后发生下肢深静脉血栓。

Objective To investigate the influencing factors of lower extremity deep venous thrombosis after glioma surgery,and to provide reference and basis for clinical prevention and treatment.Method Seventy-six patients with glioma were divided into lower extremity deep vein thrombosis group(n=21)and non-lower extremity deep vein thrombosis group(n=55)according to whether lower extremity deep vein thrombosis occurred or not after operation.The clinical characteristic of the two groups were compared,and multivariate Logistic regression analysis was used to analyze the risk factors of postoperative lower extremity deep venous thrombosis in patients with glioma.Result The proportion of patients with age>60 years,hypertension,cerebral thrombosis and lower extremity varicose veins in the lower extremity deep vein thrombosis group were higher than those in the non-lower extremity deep vein thrombosis group,the differences were statistically significant(P<0.05).Multivariate Logistic regression analysis showed that age>60 years,hypertension,cerebral thrombosis and lower extremity varicose veins were independent risk factors for postoperative lower extremity deep vein thrombosis in patients with glioma(P<0.01).Conclusion Age>60 years,hypertension,cerebral thrombosis and lower extremity varicose veins are the independent risk factors of lower extremity deep vein thrombosis in patients with glioma after operation.Relevant clinical measures should be formulated according to the above factors to prevent the occurrence of lower extremity deep vein thrombosis in patients with glioma after operation.
