电子喉镜下高频电圈套器治疗会厌囊肿的疗效分析

Analysis on effect of high-frequency electric snare device under electronic laryngoscope in treating epiglottic cyst

目的探讨电子喉镜下高频电圈套器治疗会厌囊肿的临床疗效。方法选取2021年4月至2023年3月赣南医科大学第一附属医院收治的100例确诊并接受门诊手术治疗的会厌囊肿患者,按接诊的先后顺序纳入观察组和对照组,每组50例。观察组在门诊局部麻醉电子喉镜下应用高频电圈套器行会厌囊肿切除,对照组在门诊局部麻醉电子喉镜下应用喉组织钳行会厌囊肿揭盖术。对比两种手术方法的手术时间、术中出血量、术后24 h内视觉模拟量表(VAS)评分、疼痛持续时间、完整切除率及术后3个月复发率。结果观察组手术时间、术中出血量、术后24 h内VAS评分、疼痛持续时间明显少于对照组,完整切除率明显高于对照组,差异有统计学意义(P<0.05)。随访3个月,仅对照组2例患者复发,均为会厌谷多发囊肿,但两组复发率比较差异无统计学意义(P>0.05)。两组均无呼吸困难、大出血、会厌粘连等并发症出现。结论电子喉镜下高频电圈套器治疗会厌囊肿较常规手术方式具有手术时间较短、出血量少、术后疼痛较轻、完整切除率高等优点。

Objective To explore the clinical efficacy of high-frequency electric snare device under electronic laryngoscope in the treatment of epiglottic cyst.Methods A total of 100 patients with definitely diagnosed epiglottic cyst receiving outpatient operation treatment in the First Affiliated Hospital of Gannan Medical University from April 2021 to March 2023 were selected as the research subjects and included into the observation group and control group according to the visiting order,50 cases in each group.The observation group was treated with high-frequency electric snare for epiglottic cyst resection under electronic laryngoscope,and the control group was treated with laryngeal tissue forceps under electronic laryngoscope for uncovering operation of epiglottic cyst.The operation time,intraoperative blood loss volume,pain degree within postoperative 24 h,pain duration,complete resection rate and recurrence rate in postoperative 3 months were compared between the two operation methods.Results The operation time,intraoperative blood loss volume,VAS score within postoperative 24 h and pain duration in the observation group were significantly less than those in the control group,the complete resection rate was significantly higher than that in the control group,and the differences were statistically significant(P<0.05).The two groups were followed up for 3 months.Only 2 cases in the control group relapsed,which were epiglottic multiple cyst,and the recurrence rate had no statistical difference between the two groups(P>0.05).No dyspnea,massive bleeding and epiglottic adhesion appeared.Conclusion The high-frequency electric snare device under electronic laryngoscope for treating epiglottic cyst has the advantages of short operation time,less blood loss,postoperative light pain and high complete resection rate.
