戈舍瑞林联合宫腔镜电切术对早期子宫内膜癌患者肿瘤相关因子、卵巢功能的影响

Effect of Goserrelin combined with hysteroscopic electrotomy on tumor-related factors and ovarian function in patients with early endometrial cancer

目的:探讨戈舍瑞林联合宫腔镜电切术对早期子宫内膜癌患者肿瘤相关因子、卵巢功能的影响。方法:选取2019年10月至2022年1月我院收治的130例早期子宫内膜癌患者作为研究对象,以随机数字表法分为联合组和对照组,各65例。对照组采用宫腔镜电切术联合甲羟孕酮片治疗,联合组在对照组的基础上采用戈舍瑞林治疗。比较两组女性性功能量表(Female sexual function index,FSFI)、巴氏指数(Barthel index,BI)、卵巢功能指标、肿瘤相关因子指标、复发率、生存率。结果:治疗后,联合组FSFI、BI评分、黄体生成素(Luteinizing hormone,LH)、促卵泡刺激素(Follicle stimulating hormone,FSH)、miR-335水平均明显高于对照组(P<0.05);联合组雌二醇(Estradiol,E2)、糖类抗原125(Carbohydrate antigen 125,CA125)、人附睾蛋白4(Human epididymis protein4,HE4)、血清催乳素(Prolactin,PRL)、甲壳质酶蛋白40(Chitinase protein 40,YKL-40)水平明显低于对照组(P<0.05)。随访1 y后,联合组复发率明显低于对照组(P<0.05),两组生存率无明显差异(P>0.05)。结论:戈舍瑞林联合宫腔镜电切术治疗早期子宫内膜癌患者,可有效调控肿瘤相关因子水平、性激素水平,改善卵巢功能、性功能,降低复发风险,促进预后恢复。

Objective:To investigate the effects of Goserrelin combined with hysteroscopic electrotomy on tumor-related factors and ovarian function in patients with early endometrial cancer.Methods:One hundred and thirty patients with early endometrial cancer admitted to our hospital from October 2019 to January 2022 were divided into combination group and control group with 65 cases in each group by random number table method.The control group was treated with hysteroscopic electrosurgery combined with medroxyprogesterone tablets,and the combined group was treated with goserelin on the basis of the control group.The female sexual function index(FSFI),Barthel index(BI),ovarian function indicators,tumor related factor indicators,recurrence rate,and survival rate between two groups were compared.Results:After treatment,FSFI,BI score,Luteinizing hormone(LH),follicle stimulating hormone(FSH),miR-335 levels in the combined group were significantly higher than those in the control group(P<0.05);the levels of estradiol(E2),Carbohydrate antigen 125(CA125),Human epididymis protein-4(HE4),Prolactin(PRL),and Chitin protein 40(YKL-40)in the combination group were significantly lower than those in the control group(P<0.05).After 1 year of follow-up,the recurrence rate in the combination group was significantly lower than that in the control group(P<0.05),and there was no significant difference in survival rates between the two groups(P>0.05).Conclusion:In the treatment of patients with early endometrial cancer,Goserrelin combined with hysteroscopic electro resection can effectively regulate the levels of tumor-related factors and sex hormones,improve ovarian function and sexual function,reduce the risk of recurrence,and promote prognosis.
