噻托溴铵粉联合头孢他啶治疗慢性阻塞性肺炎的疗效

Efficacy of tiotropium bromide powder combined with ceftazidime in the treatment of chronic obstructive pneumonia

目的:探讨慢性阻塞性肺炎应用噻托溴铵粉联合头孢他啶治疗的疗效。方法:选取2021年6月至2023年5月于本院就诊的98例慢性阻塞性肺炎患者作为研究对象。采用双盲法将患者分为对照组和观察组,各49例。对照组雾化吸入噻托溴铵粉治疗;观察组在对照组基础上增加静脉注射头孢他啶治疗。两组均治疗2 w。分析对比两组的肺功能指标、血清指标以及不良反应。结果:治疗后,两组的最大呼气流量(Peak expiratory flow,PEF)和一秒率(Forced expiratory volumone second/Forced vital capacity,FEV1%)均比治疗前明显提高,且观察组的PEF和FEV1%的提高幅度明显大于对照组(P<0.05)。治疗后,两组的白细胞介素-4(Interleukin 4,IL-4)、肿瘤坏死因子-α(Tumor necrosis factor-α,TNF-α)、转化生长因子-β1(Transforming growth factor-β,TGF-β1)均比治疗前明显降低,且观察组的IL-4、TNF-α、TGF-β1水平均比对照组明显降低(P<0.05)。两组治疗期间的不良反应无明显差异(P>0.05)。结论:慢性阻塞性肺炎患者应用噻托溴铵粉联合头孢他啶治疗可改善肺功能,降低炎性损伤,且安全性较好。

Objective:To investigate the curative effect of tiotropium bromide powder combined with ceftazidime in the treatment of chronic obstructive pneumonia.Methods:A total of 98 patients with chronic obstructive pneumonia who were treated in our hospital from June 2021 to May 2023 were selected as the study objects.The patients were divided into control group and observation group with 49 cases in each group.The control group was treated with atomized inhalation of tiotropium bromide powder;The observation group was treated with intravenous ceftazidime on the basis of control group.Both groups were treated for 2 weeks.The pulmonary function indexes,serum indexes and adverse reactions of the two groups were analyzed and compared.Result:After treatment,the Peak expiratory flow(PEF)and Forced expiratory volumone second/Forced vital capacity(FEV1%)in both groups were significantly higher than before treatment.The increase of PEF and FEV1%in observation group was significantly greater than that in control group(P<0.05).After treatment,the levels of Interleukin 4(IL-4),Tumor necrosis factor-α(TNF-α),Transforming growth factor-β1(β-β)in both groups increased significantly.The levels of IL-4,TNF-αand TGF-β1 in the observation group were significantly lower than those in the control group(P<0.05).There was no significant difference in adverse reactions between the two groups during treatment(P>0.05).Conclusion:Tiotropium bromide combined with ceftazidime can improve lung function and reduce inflammatory injury in patients with chronic obstructive pneumonia with good safety.
