鼻窦炎鼻内镜术后睡眠障碍的原因及护理策略分析

Analysis of Causes and Nursing Strategies of Sleep Disorder after Endoscopic Surgery for Sinusitis

目的观察鼻窦炎鼻内镜术后睡眠障碍的原因及护理策略分析。方法将我院2021年1月至2023年1月收治的鼻窦炎患者370例作为本次研究对象,根据匹兹堡睡眠质量指数(PSQI)评分≥7分者判定为睡眠障碍病将其纳入障碍组,而另评分<7分者纳入正常组。分析患者产生术后睡眠障碍的因素并制定相应的护理策略。结果与正常组相比,障碍组患者的SAS>50分、VAS>4分、无法适应术后呼吸方式率、无法适应住院环境率均更高,而术前坚持呼吸训练占有率更低,差异有统计学意义(P<0.05)。两组存在持续口鼻分泌物率、合并糖尿病、合并高血压、合并心脏病、担心住院费用占有率,无统计学意义(P>0.05)。Logistic多因素分析接受鼻内镜治疗的鼻窦炎患者术后SAS>50分、VAS>4分、无法适应呼吸方式、住院环境均为影响睡眠障碍危险因素。结论对于鼻窦炎鼻内镜术后患者,发生术后睡眠障碍的概率相对较高,患者术后SAS>50分、VAS>4分、无法适应呼吸、住院环境、术前坚持呼吸训练占有率低均为影响睡眠障碍危险因素。应当积极地给予相关的疏导,以达到改善睡眠障碍的目的。

Objective To observe the causes of Sleep disorder after endoscopic sinus surgery for Sinusitis and analyze the nursing strategies.Methods 370 patients with Sinusitis diagnosed in our hospital(January 2021 to January 2023)were the subjects of this study.According to the Pittsburgh Sleep Quality Index(PSQI)score≥7,those with Sleep disorder were included in the disorder group,while those with score<7 were included in the normal group.To analyze the factors of postoperative Sleep disorder and formulate corresponding nursing strategies.Results Compared to the normal group,the rate of SAS>50 points,VAS>4 points,inability to adapt to the postoperative breathing mode rate,inability to adapt to the hospitalization environment rate of patients in the obstacle group were higher,while the preoperative adherence to respiratory training occupancy rate was lower,the difference was statistically significant(P<0.05).The existence of persistent oral and nasal secretion rate,combined diabetes mellitus,combined hypertension,combined heart disease,and worry about hospitalization cost occupancy in the two groups was not statistically significant(P>0.05)in the rate of persistent oral and nasal secretions between the two groups.Logistic multivariate analysis SAS>50 point,VAS>4 point,inability to adapt to postoperative breathing mode,and inability to adapt to hospital environment were all risk factors for sleep disorder in patients with sinusitis who received endoscopic treatment.Conclusions For patients with sinusitis after endoscopic sinus surgery,the incidence of postoperative sleep disorder is relatively high.SAS>50 point,VAS>4 point,inability to adapt to postoperative breathing mode,and inability to adapt to hospital environment are risk factors for sleep disorder.Therefore,in clinical practice,we should actively give relevant guidance to improve sleep disorder.
