150例慢性心衰患者中医证型、血脂水平与心功能的临床研究

Clinical Observation on Correlation Between TCM Differentiation Types,Blood Lipid of 150 Patients with Chronic Heart Failure and Cardiac Function

目的观察慢性心力衰竭患者中医证型、血脂水平与心功能的关系。方法收集我院2023年1月至2023年10月收治的150例慢性心力衰竭住院患者中医证型、血脂、心功能等信息,统计并初步分析慢性心衰患者中医证型分布规律与血脂关系。结果150例慢性心衰患者中,中医证型主要为气阴两虚型,占30.00%。NYHA分级Ⅱ级、Ⅲ级、Ⅳ级患者的pro-BNP水平逐渐升高(P<0.05);Ⅲ级患者的TC水平低于Ⅱ级、Ⅳ级患者(P<0.05);Ⅲ级、Ⅳ级患者的HDL水平低于Ⅱ级患者(P<0.05)。阳虚水泛型患者的TG水平高于气阴两虚型、气虚血瘀型患者(P<0.05),TC水平高于心肺气虚型患者(P<0.05),LDL水平高于痰饮阻肺型、气虚血瘀型患者(P<0.05)。结论慢性心衰住院患者以气阴两虚型为多。伴随着HDL下降,心衰患者心功能出现恶化。各中医证型中,阳虚水泛型心衰患者血脂水平较其他组明显异常。

Objective To observe the correlation between TCM syndromes,blood lipid and cardiac function in patients with CHF.Methods The information of TCM syndrome type,blood lipid and cardiac function of 150 patients with chronic heart failure admitted to our hospital from January 2023 to October 2023 were collected.The relationship between TCM syndrome type distribution and blood lipid in patients with chronic heart failure was statistically and preliminarily analyzed.Results Among 150 patients with CHF,the main TCM syndrome type was Qi and Yin deficiency,accounting for 30.00%.The pro-BNP levels in NYHA gradeⅡ,Ⅲ,andⅣpatients gradually increased(P<0.05);The TC level of gradeⅢpatients was lower than that of gradeⅡandⅣpatients(P<0.05);The HDL levels in gradeⅢandⅣpatients were lower than those in gradeⅡpatients(P<0.05).The TG level of patients with Yang deficiency and water generalization type was higher than that of patients with Qi and Yin deficiency type,Qi deficiency and blood stasis type(P<0.05),TC level was higher than that of patients with heart lung qi deficiency type(P<0.05),and LDL level was higher than that of patients with phlegm yin obstruction type and Qi deficiency and blood stasis type(P<0.05).Conclusions Among hospitalized patients with CHF,there are more patients with Qi and Yin deficiency syndrome.Accompanied by a decrease in high-density lipoprotein,heart failure patients experience a deterioration in heart function.Among various traditional Chinese medicine syndrome types,patients with Yang deficiency and water induced heart failure have significantly abnormal blood lipid levels compared to other groups.
