摘要
目的探讨高危型人乳头瘤病毒(HR-HPV)感染与宫颈上皮内瘤变(CIN)的相关性。方法选取2019年1月-2023年1月连云港市第一人民医院妇科收治的CIN患者124例,分别将宫颈高级别鳞状上皮内病变(HSIL)患者51例分为HSIL组,宫颈低级别鳞状上皮内病变(LSIL)患者73例分为LSIL组,选取同期进行常规体检(阴道分泌物微生态和人乳头瘤病毒基因分型检测)的宫颈正常者107名健康体检者作为对照组,比较各组临床资料差异,利用Logistic回归模型分析HR-HPV感染与阴道微生态的相关性。结果多因素结果发现,HPV感染、阴道微生态异常、白细胞介素-2(IL-2)是影响LSIL发生的危险因素(P<0.05);首次性生活年龄、HPV感染、阴道pH、阴道微生态与IL-2是影响HSIL发生的危险因素(P<0.05)。结论CIN发生发展与阴道微生态异常和HPV感染可能存在相关性。
OBJECTIVE To explore the correlation between high-risk human papillomavirus(HR-HPV)infection and cervical intraepithelial neoplasia(CIN).METHODS A total of 124 CIN patients admitted to the gynecology de-partment of the First People's Hospital of Lianyungang from Jan 2019 to Jan 2023;51 patients with cervical high-grade squamous intraepithelial lesion(HSIL)were divided into the HSIL group,73 patients with cervical low-grade squamous intraepithelial lesion(LSIL)were in the LSIL group.A total of 107 cervical normal patients who were admitted during the same period under routine physical examination(vaginal secretion microecology and HPV genotyping test)were selected as the control group.The differences in the clinical data between the two groups were compared and the correlation between HR-HPV infection and vaginal microecology was analyzed by using a Logistic regression model.RESULTS Multivariate analysis showed that HPV infection,vaginal microecolo-gy and IL-2 were risk factors for the occurrence of LSIL(P<0.05).Age at first sexual intercourse,HPV infec-tion,vaginal pH,vaginal microecology and IL-2 were risk factors affecting the occurrence of HSIL(P<0.05).CONCLUSION The occurrence and development of CIN may be related to vaginal microecological abnormalities and HPV infection.
