利脑心片联合硫酸氢氯吡格雷片对急性心肌梗死患者心肌酶谱的影响

Effect of Linaoxin tablet combined with clopidogrel hydrogen shlfate tablets on myocardial enzyme profile in patients with acute myocardial infarction

目的 探讨利脑心片联合硫酸氢氯吡格雷片对急性心肌梗死患者心肌酶谱的影响。方法 选择2019年10月—2022年10月本院收治的86例急性心肌梗死患者作为研究对象,通过黑白双色球法随机分为西药组和观察组两组,每组各43例。西药组口服硫酸氢氯吡格雷片治疗,观察组在对照组基础上加用利脑心片治疗。比较两组临床疗效和不良反应发生情况,治疗前后的心功能指标、心肌酶谱、血清学指标。结果 观察组(97.67%)比西药组(74.42%)的总有效率明显更高,差异有统计学意义(P<0.05)。治疗后两组心排血量(CO)、舒张早期与晚期最大血流比(E/A)值、左室射血分数(LVEF)水平均显著升高,观察组比西药组明显更高;左心室收缩末期内径(LVESD)水平均有所降低,观察组比西药组明显更低,差异有统计学意义(P<0.05)。治疗后两组肌酸磷酸激酶(CPK)、磷酸肌酸激酶同工酶(CK-MB)、乳酸脱氢酶(LDH)、肌钙蛋白I(CTnI)水平均显著降低,观察组比西药组显著更低,差异有统计学意义(P<0.05)。治疗后两组巨噬细胞迁移抑制因子(MIF)、生长分化因子15(GDF-15)、富含半胱氨酸蛋白61(CYR61)、新型脂肪酸结合蛋白(H-FABP)水平均明显降低,观察组比西药组明显更低,差异有统计学意义(P<0.05)。观察组不良反应发生率为9.30%(4/43),西药组不良反应发生率为6.98%(3/43),两组比较差异无统计学意义(P>0.05)。结论 急性心肌梗死患者应用利脑心片联合硫酸氢氯吡格雷片治疗效果显著,明显改善患者心功能指标、心肌酶谱、血清学指标,且安全性较高。

Objective To investigate the effect of Linaoxin tablet combined with clopidogrel hydrogen shlfate tablet on myocardial enzyme profile in patients with acute myocardial infarction.Methods A total of 86 patients with acute myocardial infarction who were treated in our hospital from October 2019 to October 2022 were enrolled as study subjects,and they were randomly divided into the western medicine group and the observation group by black and white two-color ball method,with 43 cases in each group.The western medicine group was treated with clopidogrel hydrogen shlfate tablet,and the observation group was treated with Linaoxin tablet on the basis of the control group.The clinical efficacy,occurrence of adverse reactions,cardiac function index,myocardial enzyme spectrum and serological index before and after treatment were compared between the two groups.Results The total effective rate of theobservation group(97.67%) was significantly higher than that of western medicine group(74.42%),and the difference was statistically significant(P<0.05).After treatment,cardiac output(CO),early maximal ventricular filling velocity/atrial maximal ventricular filling velocity(E/A) ratio,and left ventricular ejection fraction(LVEF) levels were significantly increased in both groups,which were significantly higher in the observation group than in the western medicine group;Left ventricular end-systolic diameter(LVESD) levels were significantly lower in the observation group than in the western medicine group,and the differences were statistically significant(P<0.05).After treatment,the levels of creatine phosphokinase(CPK),Creatinekinase-MB(CK-MB),lactic dehydrogenase(LDH) and troponin I(CTnI) in both groups were significantly decreased,and the levels in the observation group were significantly lower than those in the western medicine group,with statistical significance(P<0.05).After treatment,the levels of macrophage migration inhibitory factor(MIF),Growth differentiation factor 15(GDF-15),Cysteine protein rich 61(CYR61) and heart fatty acid binding proteins(H-FABP) were significantly decreased in both groups,and the levels in the observation group were significantly lower than those in the western medicine group,with statistical significance(P<0.05).The incidence of adverse reactions was 9.30%(4/43) in the observation group and 6.98%(3/43) in the western medicine group,and there was no statistical significance between the two groups(P>0.05).Conclusions The treatment effect of Linaoxin tablet combined with Clopidogrel hydrogen shlfate tablet in patients with acute myocardial infarction is significant,which can significantly improve the cardiac function index,myocardial enzyme profile and serological index of patients,which has high safety.
