摘要
目的 探讨一种改良上颌窦内提升术在临床上的应用。方法 选择2019年6月—2020年12月上颌后牙缺失的患者36例作为研究对象,术前CBCT测量剩余骨高度(RBH)5~6 mm,采用骨挤压器应用穿牙槽嵴技术行上颌窦内提升术,植入40颗种植体,不使用骨移植材料。术后立即拍片观察上颌窦底黏膜的完整性及测量上颌窦底提升的高度。术后6个月行种植二期手术及种植牙冠修复。结果 上颌窦提升高度:2~3 mm;种植体直径:4~5 mm;种植体高度:8~9 mm。术后随访1~2年,种植体骨结合良好,种植体存留率100%。结论 通过骨挤压器行上颌窦内提升术,手术创伤小,操作简单,手术费用少,获得较满意的临床效果。
Objective To discuss the application of improved maxillary sinus augmentation surgery in clinical practice.Methods A total of 36 patients with absence of maxillary posterior teeth during June 2019 and December 2020 were selected as the study subjects.The residual bone height(RBH)was 5~6 mm measured by CBCT preoperative.Intramaxillary sinus elevation was performed with bone extruder and transalveolar ridge technique,40 implant were implanted,no bone graft material was used.The integrity of maxillary sinus mucosa and the elevation of maxillary sinus were measured immediately after operation.The second stage of implant surgery and dental crown restoration were performed 6 months after surgery.Results The lifting height of maxillary sinus was about 2~3 mm;The diameter of implant was 4~5 mm;The height of implant was 8~9mm.Postoperative follow-up lasted for 1 to 2 years,implants and the bone combined well,and survival rate of implant was 100%.Conclusions The maxillary sinus augmentation by bone extruder is a surgical method of minimal surgical trauma,simple operation,and low operation cost,which can obtain satisfactory clinical effect.
