PNF技术对脑卒中患者膝关节过伸及下肢运动功能的影响

摘要 目的:研究PNF技术对脑卒中患者膝关节过伸及下肢运动功能的影响。方法:选择蚌埠医学院第一附属医院2021年6月~2022年6月在康复医学科住院进行康复治疗的脑卒中患者60例,随机分为对照组和试验组各30例,对照组采用常规康复治疗,试验组在此基础上增加PNF等张组合、节律性稳定及动态反转技术加强膝关节周围肌肉力量及本体感觉训练,改善患者膝关节控制及下肢运动功能。两组患者通过测量膝关节伸展角度判断治疗有效率,以及采用FMA-L评分量表、BBS评分量表和计算10m最大步行速度、平均步长结果来评价脑卒中患者下肢运动功能的改善。结果:治疗3周后,两组患者在膝过伸角度、FMA-L评分、BBS评分、10m最大步行速度和平均步长评估指标中分数均有显著提高,其中试验组改善程度显著优于对照组,差异有统计学意义(P<0.01)。结论:对脑卒中后偏瘫患者进行PNF等张组合、节律性稳定及动态反转技术可增强膝关节周围肌肉力量及本体感觉,改善膝关节的稳定性及控制能力,提高下肢的运动功能。 Objective:To examine the effects of proprioceptive neuromuscular facilitation(PNF)techniques on knee hyperextension and lower limb motor function in stroke patients.Methods:Sixty stroke patients who underwent rehabilitation treatment in Rehabilitation Medicine Department of the First Affiliated Hospital of Bengbu Medical College from June 2021 to June 2022 were selected and randomly divided into control group(n=30)and experimental group(n=30).The control group received standard rehabilitation treatment,while the experimental group received isotonic PNF combinations,rhythm stabilization,and dynamic reversal techniques in addition to standard rehabilitation treatment.These techniques aimed to strengthen the muscles around the knee joint and provide proprioception training,to improve knee joint control and lower limb motor function.Two groups of patients were evaluated for treatment effectiveness by measuring the knee joint extension angle,and the improvement of lower limb motor function in stroke patients was evaluated using the FMA-L score scale,BBS score scale,and the results of calculating the maximum walking speed and average stride length of 10m.Results:After three weeks of treatment,both groups showed significant improvements in knee hyperextension angle,FMA-L score,BBS score,10-meter maximum walking speed and average step length.Moreover,the experimental group showed significantly greater improvement than the control group(P<0.01).Conclusion:Isotonic PNF combination,rhythmic stabilization,and dynamic inversion techniques can enhance muscle strength and proprioception around the knee joint,improve knee joint stability and control,and improve lower limb motor function in patients with hemiplegia after stroke.
