慢性阻塞性肺疾病相关影响因素及炎症因子表达

摘要 目的:探究慢性阻塞性肺疾病(Chronic obstructive pulmonary diseases,COPD)影响因素及炎性状态,为慢性阻塞性肺疾病防治提供参考。方法:选取2020年6月至2020年12月在赣南医科大学第一附属医院呼吸内科住院的COPD患者共67例为病例组,以同期在医院体检中心检测且无呼吸系统疾病的体检者72例为对照组,问卷调查收集相关信息。采用单因素χ^(2)检验与非条件多因素Logistic回归分析COPD患病的危险因素;采用t检验比较2组血清炎症因子水平的差异。结果:病例组与对照组在性别、家庭月平均收入、职业情况、吸烟、锻炼、睡眠时间、睡眠质量、是否居住公路旁等因素差异有统计学意义(P<0.05);多因素Logistic回归分析结果显示家庭月平均收入低(OR=5.126,95%CI:2.113~12.435)、吸烟(OR=4.103,95%CI:1.113~15.122)、睡眠时间短(OR=27.279,95%CI:7.486~99.409)是COPD的危险因素,睡眠质量好(OR=0.128,95%CI:0.042~0.395)为COPD的保护因素;病例组血清TNF-α、IL-8、MCP-1水平均高于对照组,差异有统计学意义(P<0.05)。结论:家庭月平均收入低、吸烟、睡眠时间短是COPD患病的危险因素,外周血炎性因子可能与COPD疾病发展有关。 Objective:To explore the influencing factors and inflammatory state with chronic obstructive pulmonary disease(COPD)and provide a reference for the prevention and treatment of COPD.Methods:A total of 67 patients with COPD hospitalized in the Department of Respiratory Medicine,The First Affiliated Hospital of Gannan Medical University from June 2020 to December 2020 were selected as COPD group,and 72 individuals who underwent health checks in the same hospital during the same period and diagnosed not having COPD were chosen as the control group.Relevant data were collected through questionnaires.Univariateχ^(2) test and unconditional multivariate logistic regression were used to analyze the risk factors of COPD.A t-test was used to compare the serum levels of oxidative stress and inflammatory markers between the two groups.Results:There were statistically significant differences between the two groups in gender,monthly average family income,occupational status,smoking,exercise,sleep time,sleep quality and living on the side of road or not(P<0.05).Multivariate Logistic regression analysis showed that the lower the average monthly household income(OR=5.126,95%CI:2.113-12.435),the smoking(OR=4.103,95%CI:1.113-15.122)and short sleep duration(OR=27.279,95%CI:7.486-99.409)were risk factors for COPD,and good sleep quality(OR=0.128,95%CI:0.042-0.395)was a protective factor for COPD.The serum levels of TNF-α,IL-8 and MCP-1 in the COPD group were higher than those in the control group,and the differences were statistically significant(P<0.05)Conclusion:A lower average monthly family income,smoking,and short sleep duration are significant risk factors for COPD.The levels of inflammatory markers in the peripheral blood may be related to the progression of COPD.
