摘要
目的探讨阶段性的持续健康教育干预在产后压力性尿失禁患者盆底康复训练中的应用效果。方法选用复旦大学附属妇产科医院2019年3月至2022年3月收治的产后压力性尿失禁患者98例作为研究对象,根据干预方式的不同分为对照组48例(采用常规护理干预)和观察组50例(采用常规护理+阶段性持续健康教育干预)。对比两组国际尿失禁问卷表(The International Consultation on Incontinence Questionnaire Short Form,ICI-Q-SF)评分、盆底肌力、抑郁自评量表(Self-Rating Depression Scale,SDS)、焦虑自评量表(Self-Rating Anxiety Scale,SAS)评分、生活质量。结果干预后,两组ICI-Q-SF评分都降低,且观察组的ICI-Q-SF评分低于对照组,差异均有统计学意义(均P<0.05)。干预后,两组盆底肌力都升高,且观察组的盆底肌力高于对照组,差异均有统计学意义(均P<0.05)。干预后,两组SAS和SDS评分都降低,且观察组的SAS和SDS评分低于对照组,差异均有统计学意义(均P<0.05)。干预后,两组I-QOL评分都升高,且观察组的I-QOL评分高于对照组,差异均有统计学意义(均P<0.05)。结论阶段性持续健康教育干预产后压力性尿失禁患者有助于其缓解尿失禁临床表现,改善焦虑抑郁心理状态,提高生活质量。
Objective To explore the application effect of staged continuous health education intervention in pelvic floor rehabilitation training for patients with postpartum stress urinary incontinence.Methods A total of 98 patients with postpartum stress urinary incontinence admitted to the Obstetrics and Gynecology Hospital of Fudan University from 2019 to 2022 were selected as the research objects.According to the different intervention methods,they were divided into the control group(48 cases were treated with routine nursing intervention)and the observation group(50 cases were treated with routine nursing combined with staged continuous health education intervention).The scores of International Consultation on Incontinence Questionnaire Short Form(ICI-Q-SF),pelvic floor muscle strength,Self-Rating Depression Scale(SDS),Self-Rating Anxiety Scale(SAS)and quality of life were compared between the two groups.Results After intervention,the ICI-Q-SF scores of the two groups were reduced,and the ICI-Q-SF score of the observation group was lower than that of the control group,and the differences were statistically significant(P<0.05).After intervention,the pelvic floor muscle strength of the two groups increased,and the pelvic floor muscle strength of the observation group was higher than that of the control group,and the differences were statistically significant(P<0.05).After intervention,the SAS and SDS scores of the two groups decreased,and the SAS and SDS scores of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).After intervention,the I-QOL scores of the two groups increased,and the I-QOL score of the observation group was higher than that of the control group,and the differences were statistically significant(P<0.05).Conclusions Stage continuous health education intervention for patients with postpartum stress urinary incontinence is helpful to relieve the clinical manifestations of urinary incontinence,improve the psychological state of anxiety and depression,and improve the quality of life.
