系统性红斑狼疮患者外周血自然杀伤T细胞免疫球蛋白样转录物2水平变化和临床意义

The clinical significance of the immunogolobulin-like transcript 2 expression in natural killer T cells from patients with systemic lupus erythematosus

摘要 目的探讨系统性红斑狼疮(SLE)患者外周血自然杀伤T细胞(NKT)细胞的免疫球蛋白样转录物2(ILT2)表达水平和疾病活动的相关性及与感染的关系。方法选取2022年1月至7月于福建医科大学省立临床医学院风湿免疫科住院的SLE患者50例,健康对照者24名,流式细胞术检测NK、NKT细胞和ILT2+NK、ILT2+NKT细胞的水平,并结合临床资料分析。随访其中6例SLE合并感染患者,观察感染控制后ILT2+NKT细胞的水平的变化。采用t检验及Pearson相关性分析统计数据。结果SLE患者外周血中NKT细胞比率较健康对照组降低[(1.5±1.7)%与(2.5±1.2)%,t=2.54,P=0.013]。SLE患者的ILT2+NKT细胞的比率显著增加[(70±17)%与(54±20)%,t=3.43,P=0.001]。ILT2+NKT细胞的比率与SLEDAI(r=0.28,P=0.046)、血清肌酐(r=0.30,P=0.031)呈正相关,与补体C3呈负相关(r=-0.32,P=0.022)。存在感染或感染病史的SLE患者ILT2+NKT细胞的表达水平高于无感染的SLE患者[(79.2±12.4)%与(65.1±1.0)%,t=2.84,P=0.007]。6例合并感染的SLE患者,经过治疗后,复测外周血ILT2+NKT细胞较治疗前下降[(85±7)%与(69±6)%,t=3.45,P=0.001]。结论SLE患者NKT细胞的ILT2高表达可能参与了SLE疾病的发生发展,并可能与感染有关。 Objective To investigate the clinical significance of the immunoglobulin-like transcript2(ILT2)expression in natural killer T cells from patients with systemic lupus erythematosus(SLE).Methods Fifty patients with SLE from January 2022 to July 2022 in Shengli Clinical Medical College of Fujian Medical University and 24 healthy controls were enrolled.The expression of NK cells,NKT cells,ILT2+NK cells,and ILT2+NKT cells were detected by flow cytometry.The changes of ILT2+NKT cells were observed in 6 patients with infection after treatment.Data were analyzed by t test and Pearson correlation analysis.Results The expression of NKT cells in the peripheral blood of patients was lower than that in the controls[(1.5±1.7)%vs.(2.5±1.2)%,t=2.54,P=0.013].The level of ILT2+NKT cells was obviously higher[(70±17)%vs.(54±20)%,t=3.43,P=0.001].The level of ILT2+NKT cells were positively correlated with SLEDAI(r=0.28,P=0.046),serum creatinine(r=0.30,P=0.031),and negatively correlated with complement C3(r=-0.32,P=0.022).ILT2+NKT cells significantly increased in SLE patients with infection[(79.2±12.4)%vs.(65.1±1.0)%,t=2.84,P=0.007],and decreased after anti-infection treatment[(85±7)%vs.(69±6)%,t=3.45,P=0.001].Conclusion The expression of peripheral ILT2+NKT cells in SLE patients is higher than that in healthy controls,and is related to disease activity indicators.The expression of ILT2+NKT cells in SLE patients infection is increased significantly,and decreased after anti-infection treatment.It is suggested that the overexpression of ILT2 in NKT cells may participate in the occurrence and development of SLE diseases,and may be related to the occurrence of infection.
