维生素D_(2)注射液对80岁以上男性维生素D缺乏患者血清25-羟基维生素D和免疫功能的影响

Influences of vitamin D_(2)injection on serum 25-hydroxyvitmin D and immune function in men with vitamin D deficiency aged 80 and over

摘要 目的研究维生素D_(2)注射液对80岁以上老年男性维生素D缺乏患者血清25-羟基维生素D[25(OH)D]和免疫功能的影响。方法选择48例年龄80岁以上男性维生素D缺乏[血清25(OH)D<20μg·L^(-1)]患者,给予维生素D_(2)注射液(60万单位,肌内注射,每月1次)治疗,直至血清25(OH)D≥30μg·L^(-1)后停用。检测治疗前后血清25(OH)D_(2)、25(OH)D_(3),T淋巴细胞亚群(包括CD3^(+)、CD4^(+)、CD8^(+))和CD19^(+)B细胞、血钙、血磷等水平,并观察不良反应发生情况。结果共40例患者完成研究,25(OH)D达标用时(6.2±1.4)个月。与治疗前比较,治疗后25(OH)D_(2)升高(27.42±5.47)μg·L^(-1),25(OH)D_(3)降低(5.25±3.38)μg·L^(-1),25(OH)D升高(22.45±6.15)μg·L^(-1),治疗前后比较均有非常显著差异(P<0.01)。CD4^(+)T细胞升高(3.71±6.26)%,CD4^(+)/CD8^(+)增高0.31±0.93,CD19^(+)B细胞升高(1.50±3.31)%,与治疗前比较均有显著差异(P<0.05)。血CD3^(+)T细胞、丙氨酸转氨酶、血糖、肌酐、血钙、血磷均无显著变化(P>0.05),无严重不良反应发生。结论维生素D_(2)注射液可提高80岁以上老年男性维生素D缺乏患者的25(OH)D_(2)水平,并可能调节其免疫功能。 AIM To evaluate the influences of vitamin D,injection on serum 25-hydroxyvitmin D(25(OH)D)and immune function in men with vitamin D deficiency aged 80 and over.METHODS A total of 48 men with vitamin D deficiency aged 80 and over were given intramuscular vitamin D,600000 IU every month until the serum 25(0H)D reached and over 30μg·L^(-1).The levels of serum 25(OH)D_(2),25(OH)D_(3),T lymphocyte cells(including CD3^(+)、CD4^(+)、CD8^(+)),B lymphocyte cells(CD19^(+)),and blood calcium and phosphorus were detected before and after treatment.And adverse reactions were observed.RESULTS A total of 40 patients completed the study.It took(6.2±1.4)months for 25(OH)D to reach targeted serum concentration.Compared with those before treatment,the serum levels of 25(OH)D_(2)after the treatment increased by(27.42±5.47)μg·L^(-1),25(OH)D,decreased by(5.25±3.38)μg·L^(-1),and the total 25(0H)D increased by(22.45±6.15)μg·L^(-1)(all P<0.01).And the serum levels of CD4^(+)T cells increased by(3.71±6.26)%,CD4^(+)/CD8^(+)increased 0.31±0.93,and CD19 B cell increased by(1.50±3.31)%(all P<0.05).There were no significant differences in serum CD3^(+)T cells,alanine aminotransferase,blood glucose,creatinine,calcium,and phosphorus before and after the treatment(P>0.05).No severe adverse drug reactions occurred during the treatment.CONCLUSIONVitamin D,injection is helpful for increasing serum 25(OH)D levels and regulating immune function in men with vitamin D deficiency aged 80 and over.
