加味黄芪桂枝五物汤联合中药足浴治疗糖尿病周围神经病变的效果及对电流感觉阈值的影响

Effect of modified Huangqi Guizhi Wuwu decoction combined with traditional Chinese medicine foot bath on diabetic peripheral neuropathy and its influence on current perception threshold

目的研究加味黄芪桂枝五物汤联合中药足浴治疗糖尿病周围神经病变的效果及对电流感觉阈值(CPT)的影响。方法选取我院2020年1月至2021年1月收治的100例糖尿病周围神经病变患者,以抛硬币法随机将其分为对照组和观察组,各50例。对照组采用常规甲钴胺治疗,观察组采用加味黄芪桂枝五物汤联合中药足浴治疗。比较两组的治疗效果。结果观察组的治疗总有效率高于对照组(P<0.05)。观察组的足部溃疡肉芽组织持续时间和足部溃疡愈合时间短于对照组,视觉模拟评分法(VAS)评分低于对照组(P<0.05)。治疗后,两组的神经症状、神经反射、感觉功能评分及多伦多临床神经病变评分(TCSS)总分均降低,且观察组低于对照组(P<0.05)。观察组的正中神经、尺神经、腓浅神经及腓肠神经CPT在5、250、2000 Hz频段均低于对照组(P<0.05)。治疗后,两组的肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)及C反应蛋白(CRP)水平均降低,且观察组低于对照组(P<0.05)。结论加味黄芪桂枝五物汤联合中药足浴治疗糖尿病周围神经病变患者临床效果显著。

Objective To study the effect of modified Huangqi Guizhi Wuwu decoction combined with traditional Chinese medicine foot bath in the treatment of diabetic peripheral neuropathy and its influence on current perception threshold(CPT).Methods A total of 100 patients with diabetic peripheral neuropathy admitted in our hospital from January 2020 to January 2021 were selected and randomly divided into control group and observation group by coin tossing method,with 50 cases in each group.The control group was treated with conventional mecobalamin,and the observation group was treated with modified Huangqi Guizhi Wuwu decoction combined with traditional Chinese medicine foot bath.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05).The duration of foot ulcer granulation tissue and foot ulcer healing time in the observation group were shorter than those in the control group,and the Visual Analogue Score(VAS)score was lower than that in the control group(P<0.05).After treatment,the neurological symptoms,nerve reflex,sensory function scores and Toronto Clinical Scoring System(TCSS)total scores of the two groups decreased,and those in the observation group were lower than the control group(P<0.05).The CPT of median nerve,ulnar nerve,superficial peroneal nerve and sural nerve in the observation group were lower than those in the control group at 5,250 and 2000 Hz(P<0.05).After treatment,the levels of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and C reactive protein(CRP)in the two groups decreased,and those in the observation group were lower than the control group(P<0.05).Conclusion The modified Huangqi Guizhi Wuwu decoction combined with traditional Chinese medicine foot bath has a significant clinical effect on patients with diabetic peripheral neuropathy.
