目的构建简易版糖尿病足风险筛查量表,旨在为糖尿病患者提供一份简单、快速的筛查工具。方法成立课题研究小组,于2023年1月通过文献回顾及对15例患者进行访谈,形成初始量表条目池,课题研究小组进行讨论并对量表条目池进行归类和整理。2023年3月至5月运用德尔菲法对量表进行修改调整并进行临床实践。结果两轮专家函询问卷有效回收率为100.0%,第一轮专家的权威系数为0.85,第二轮专家的权威系数为0.88,均>0.7,专家积极程度和权威程度较高,此次函询具有较高的可信度。经过函询修改后最终形成包含4个维度、10个条目的简易版糖尿病足风险筛查量表。将专家函询后的量表投入临床应用,在30例住院糖尿病患者中筛查出糖尿病高危足12例,运用振动感觉阈值(VPT)和踝肱血压指数(ABI)进行第二次筛查,筛查出11例糖尿病足中风险患者,量表与VPT、ABI筛查糖尿病高危足的共同阳性率为91.7%。结论编制的简易版糖尿病足风险筛查量表经过严格的专家函询和临床实践,可以用于糖尿病患者足部健康现状的评估,为临床医务人员提供简单、快速的糖尿病足风险筛查工具。

Objective To construct a simple version of the diabetic foot risk screening scale,which aims to provide a simple and rapid screening tool for diabetic patients.Methods The research group was established and an initial pool of scale items was formed through literature review and interviews with 15 patients in January 2023.Through discussions within the research group,the scale item pool was classified and organized.From March to May 2023,the Delphi method was used to modify and adjust the scale and conduct clinical practice.Results The effective recovery rate of the two rounds of expert consultation questionnaire was 100.0%.The authority coefficient of the first round of experts was 0.85,and the authority coefficient of the second round of experts was 0.88,both>0.7,the positive degree and authoritative degree of experts was high,and the consultation had high credibility.After consultation and revision,a simple version of the diabetic foot risk screening scale containing 4 dimensions and 10 items was finally formed.The scale after expert consultation was put into clinical application,and 12 cases of high-risk diabetic foot were screened in 30 hospitalized diabetic patients,the vibration perception threshold(VPT)and ankle brachial index(ABI)were used for the second screening,and 11 patients with moderate risk of diabetic foot were screened out,the common positive rate of the scale,VPT and ABI in screening high-risk diabetic foot was 91.7%.Conclusion The simple version of diabetic foot risk screening scale can be used to evaluate the foot health status of diabetic patients after strict expert consultation and clinical practice,and provide a simple and rapid diabetic foot risk screening tool for clinical medical staff.
