中药超声离子导入联合雷火灸对晚期肺癌患者癌性疼痛的治疗效果研究

Effect of ultrasound transdermal herb delivery combined with thunder-fire moxibustion in relieving pain degree of advanced lung cancer patients

目的观察中药超声离子导入联合雷火灸对晚期肺癌患者癌性疼痛的治疗效果。方法选取2020年10月至2022年10月收治的晚期肺癌癌性疼痛患者76例为研究对象,根据随机单盲原则分为2组,对照组38例予西医常规治疗,治疗组38例在对照组的基础上加用中药超声离子导入联合雷火灸治疗。2组均治疗14天后统计疗效,比较2组治疗前后疼痛程度、疼痛因子[包括血清β内啡肽(β-EP)、5-羟色胺(5-HT)、内皮素-1(ET-1)及P物质(SP)]及癌症患者生命意义感量表(C-MiLS)评分变化情况。结果治疗组总有效率94.74%(36/38),对照组总有效率73.68%(28/38),治疗组总有效率高于对照组(P<0.05)。与本组治疗前比较,2组治疗后疼痛程度均减轻(P<0.05),且治疗组治疗后疼痛程度轻于对照组(P<0.05)。与本组治疗前比较,2组治疗后血清β-EP水平均升高(P<0.05),5-HT、ET-1及SP水平均降低(P<0.05),且治疗组治疗后β-EP水平高于对照组(P<0.05),5-HT、ET-1及SP水平均低于对照组(P<0.05)。与本组治疗前比较,2组治疗后C-MiLS评分均升高(P<0.05),且治疗组治疗后C-MiLS评分高于对照组(P<0.05)。结论中药超声离子导入联合雷火灸治疗晚期肺癌患者疼痛症状疗效显著,不仅能调节疼痛因子表达,降低疼痛程度,而且还可促进患者生命意义感提高。

Objective To analyze the effect of ultrasound transdermal herb delivery combined with thunder-fire moxibustion in relieving pain degree of advanced lung cancer(ALC)patients.Methods This randomised,single-blind study included 76 ALC patients admitted to the hospital from October 2020 to October 2022.They were randomized 1:1 assigned to the control group(routine intervention)or the observation group(ultrasound transdermal herb delivery combined with thunder-fire moxibustion and routine intervention).The intervention effects was statistically analyzed after 14-day treatment,the changes in pain degree,pain mediators including serumβ-endorphin(β-EP),5-hydroxytryptamine(5-HT),endothelin-1(ET-1)and substance P(SP),and cancer patients'sense of meaning in life scale(C-MiLS)score were observed.Results After intervention,the total effective rate in the observation group was higher than that of control group(94.74%[36/38]vs 73.68%[28/38],P<0.05).The pain degree in the both groups was significantly alleviate(P<0.05),the alleviate in the observation group was more pronounced than that of the control group(P<0.05).Significantly higherβ-EP and significantly lower 5-HT,ET-1 and SP were detected in the both groups(P<0.05),and the improvements were more common in the observation group than in the control group(P<0.05).After intervention,the C-MiLS score in the both groups was significantly increased(P<0.05),which was more higher in the observation group than in the control group(P<0.05).Conclusion The application of ultrasound transdermal herb delivery combined with thunder-fire moxibustion in ALC patients has significant therapeutic effects,it can not only inhibit the expression of pain factors and reduce the degree of pain,but also furtherly promote the improvement of the patient's sense of meaning in life.
