结直肠腺癌中错配修复蛋白缺失情况、p53表达情况及临床病理分析的研究

Deletion of Mismatch Repair Proteins and p53 Expression and Their Correlation with Clinicopathological Features in Colorectal Adenocarcinoma

目的 探讨结直肠腺癌中错配修复蛋白缺失情况、p53表达情况及其与患者临床特征之间的关联性。方法 回顾性分析2021年1月至2022年1月眉山市人民医院收集的经过病理学检查确诊为结直肠腺癌患者282例的临床资料,分析错配修复蛋白缺失情况、p53表达情况及其与患者临床特征之间的关系;分析p53表达情况与错配修复蛋白缺失的相关性。结果 免疫组化发现,MLH1、MSH2、MSH6、PMS2的缺失数分别为32、21、17、39例,而p53的(+)、(-)、(+/-)表达分别为142、95、45例。部分错配修复蛋白的缺失与肿瘤最大径、肿瘤分化程度、T分期、有无淋巴结转移、有无神经累及、有无脉管累及存在关联(P<0.05);p53的表达与患者的肿瘤最大径、肿瘤分化程度、T分期、有无淋巴结转移、有无神经累及、有无脉管累及存在关联(P<0.05);结直肠腺癌中,p53与MLH1、PMS2存在相关性(P<0.05)。结论 结直肠腺癌患者的错配修复蛋白缺失情况、p53表达情况与患者临床病理特征存在关联性,且错配修复蛋白缺失情况和p53表达情况同样存在关联性。

Objective To investigate and analyze the deletion of mismatch repair proteins and p53 expression in colorectal adenocarcinoma and their correlation with the clinical characteristics of patients.Methods The clinical data of 282 patients diagnosed with colorectal adenocarcinoma by pathological examination in Meishan City People's Hospital from January 2021 to January 2022 were retrospectively analyzed.The deletion of mismatch repair proteins and the expression of p53 were analyzed,as well as their correlation with the clinical characteristics of patients.The correlation between the expression of p53 and the deletion of mismatch repair proteins was analyzed.Results Immunohistochemistry showed that the deletions of MLH1,MSH2,MSH6 and PMS2 occurred in 32,21,17 and 39 cases,respectively,while the expression of p53(+),(-)and(+/-)occurred in 142,95 and 45 cases,respectively.The deletion of some mismatch repair proteins was associated with the maximum diameter of tumor,tumor differentiation,T staging,lymph node metastasis,nerve involvement and vascular involvement(P<0.05).The expression of p53 was correlated with the maximum diameter of tumor,tumor differentiation,T staging,lymph node metastasis,nerve involvement and vascular involvement(P<0.05).The expression of p53 was correlated with the deletion of MLH1 and PMS2 in colorectal adenocarcinoma(P<0.05).Conclusion The deletion of mismatch repair proteins and the expression of p53 in patients with colorectal adenocarcinoma are correlated with the clinicopathological characteristics of patients,and the deletion of mismatch repair proteins and the expression of p53 are also correlated.
