摘要
目的通过检测程序性死亡配体-1(PD-L1)和OX40L在NK/T细胞淋巴瘤中的表达,分析PD-L1及OX40L的表达与NK/T细胞淋巴瘤的临床分期、LDH水平、β2微球蛋白水平、Ki-67等之间的关系,通过二者表达水平的相关性分析探讨二者在肿瘤局部微环境的表达关系。方法应用Envision法检测35例NK/T细胞淋巴瘤组织(观察组)和17例慢性鼻-鼻窦炎组织(对照组)中PD-L1及OX40L的表达情况,探讨NK/T细胞淋巴瘤的临床病理特征与两种因素之间的相互联系,并分析这两种因素表达水平之间的相关性。利用SPSS 26.0软件对所得数据进行综合统计处理。组间差异的比较采用χ^(2)检验,相关性分析采用非参数Spearman秩相关分析,以P<0.05为差异有统计学意义。结果(1)PD-L1在NK/T细胞淋巴瘤组织(74.3%)中的表达与在慢性鼻-鼻窦炎伴鼻息肉组织(76.5%)中的表达,P>0.05表示差异无统计学意义。NK/T细胞淋巴瘤组织中PD-L1的阳性表达的关联因素常见为临床分期(Ann Arbor分期)、LDH水平、Ki-67(P<0.05),而与患者性别、年龄、临床症状、β2微球蛋白关联不明显;(2)OX40L在NK/T细胞淋巴瘤组织(37.1%)中的表达低于对照组(88.2%),差异有统计学意义。NK/T细胞淋巴瘤组织中OX40L的阳性表达与临床分期有关(P<0.05),与患者性别、年龄、临床症状、β2微球蛋白水平、LDH水平、Ki-67无明显相关性;(3)NK/T细胞淋巴瘤组织中PD-L1、OX40L的表达呈负相关(r=-0.630,P<0.05);(4)PD-L1与NK/T细胞淋巴瘤的临床疗效呈负相关(r=-0.344,P<0.05),而OX40L与NK/T细胞淋巴瘤的临床疗效无相关性。结论PD-L1的高表达及OX40L的低表达可能参与NK/T细胞瘤的发展。PD-L1与临床疗效的相关性,进而说明PD-1/PD-L1信号通路可能成为NK/T细胞淋巴瘤免疫治疗的新靶点。
Objective By detecting the expression of programmed death ligand-1(PD-L1)and OX40L in NK/T cell lymphoma,the relationship between the expression of PD-L1 and OX40L and the clinical stage,LDH level,β2 MG level,Ki-67 and so on was analyzed.The relationship between the expression levels of the two in the local tumor microenvironment was investigated by correlation analysis.Methods The expression of PD-L1 and OX40L in 35 cases of NK/T cell lymphoma tissues(observation group)and 17 cases of chronic rhinosinusitis tissues(control group)were detected by Envision method and the relationship between the two and clinicopathological parameters of NK/T cell lymphoma and the correlation of their expression were analyzed.SPSS26.0 software was used for statistical analysis of the data.χ^(2) test was used to compare the differences between groups,and Spearman rank correlation analysis was used for correlation analysis.The difference was statistically significant(P<0.05).Results(1)The expression of PD-L1 in NK/T cell lymphoma tissues(74.3%)and chronic rhinosinusitis with nasal polyp tissues(76.5%)were not statistically significant(P>0.05).In the NKTL tissues,the expression of PD-L1 is related to clinical stage,LDH and Ki-67(P<0.05),but there is no relation with gender,age,clinical symptoms andβ2-MG.(2)The levels of OX40L expression in NKTL tissues(37.1%)were significantly lower than the control group(88.2%);and there is a statistically significant difference between two groups(P<0.05).In the NKTL tissues,the expression of OX40L is related to the clinical stage(P<0.05),but there is no relation with the gender,age,clinical symptoms,β2-MG,LDH and Ki-67.(3)The expression of PD-L1 has negative correlation with OX40L in the NKTL tissues(r=-0.630,P<0.05).(4)The expression of PD-L1 has negative correlation with clinical efficacy in the NKTL tissues(r=-0.344,P<0.05),but the expression of OX40L has no correlation with clinical efficacy.Conclusion The high expression of PD-L1 and the low expression of OX40L may be involved in the development of NK/T cell tumors.The correlation between PD-L1 and clinical efficacy further indicates that PD-1/PD-L1 signaling pathway may be a new target for immunotherapy of NK/T cell lymphoma.
