汗疱疹样型类天疱疮1例

Dyshidrosiform Pemphigoid:A Case Report

患者女,56岁,足部反复出现红斑水疱2个月,加重伴手部红斑水疱2周。组织病理示:皮肤组织角质层下可见水疱,棘层肥厚增生伴水肿,表皮下裂隙形成。直接免疫荧光示:基底膜带有C3呈线性沉积。血清抗BP180抗体:36.29 U/mL。诊断:汗疱疹样型类天疱疮(DP)。予泼尼松片、盐酸米诺环素胶囊、烟酰胺片联合卤米松乳膏治疗后皮损逐渐消退,随访1年未再复发。

A 56-year-old female presented with recurrent erythematous blisters on the feet for 2 months,aggravated with erythematous blisters on the hands for 2 weeks.Skin histopathology revealed the presence of blisters in the subcorneal layer,hyperplasia of the spinous layer with edema,and formation of fissures in the subepidermal layer.Direct immunofluorescence showed linear deposition of C3 in the basement membrane zone.The level of anti-BP180 antibody was 36.29 U/mL.The diagnosis of dyshidrosiform pemphigoid was made.The skin lesions gradually subsided after treatment with prednisone,minocycline,nicotinamide and halometasone cream.And there was no recurrence in the follow-up after one year.
