药物涂层球囊和药物洗脱支架治疗急性心肌梗死的对比研究

Drug Coated Balloon and Drug Eluting Stent in the Treatment of Acute Myocardial Infarction

目的:对比药物涂层球囊(drug-coated balloon,DCB)和药物洗脱支架(drug-eluting stent,DES)治疗急性心肌梗死(acute myocardial infarction,AMI)的临床疗效和安全性,为实践中选择合理的治疗方案提供参考。方法:选取2019年3月~2022年7月接受冠状动脉介入治疗的急性心肌梗死患者63例进行回顾性分析。按照冠状动脉介入治疗的方案分为DCB组和DES组。收集并比较所有患者冠状动脉介入术前的基线资料、介入手术相关信息、介入术后短期和1年随访期间的复查结果和主要不良心血管事件(MACE)。结果共有63例AMI患者纳入最终分析,其中DCB组30例,DES组33例。两组患者术后即刻的心肌梗死溶栓(thrombolysis inmyocardialinfarction,TIMI)血流分级、心电图ST段回落不良的差异、住院期间两组患者的并发症发生率差异以及1年随访期间MACE发生率的差异均无统计学意义(P>0.05)。但DCB组晚期管腔丢失优于DES组。结论:药物洗脱支架和药物涂层球囊治疗急性心肌梗死的疗效和安全性相似,DCB在改善晚期管腔丢失方面可能优于DES。

Objective:To compare the clinical efficacy and safety of drug coated balloon(DCB)and drug eluting stent(DES)in the treatment of acute myocardial infarction(AMI),and provide reference for selecting a reasonable treatment plan in practice.Methods:A retrospective analysis was conducted on 63 patients with acute myocardial infarction who received coronary intervention treatment from March 2019 to July 2022.Divided into DCB group and DES group according to the coronary intervention treatment plan.Collect and compare baseline data,intervention related information,short-term and 1-year follow-up results,and major adverse cardiovascular events(MACE)of all patients before coronary intervention.Results:A total of 63 AMI patients were included in the final analysis,including 30 in the DCB group and 33 in the DES group.There was no statistically significant difference in the immediate thrombolysis in myocardial infarction(TIMI)blood flow grading,ST segment poor regression on electrocardiogram,incidence of complications during hospitalization,and incidence of MACE during 1-year follow-up between the two groups of patients after surgery(P>0.05).But the late stage luminal loss in the DCB group was better than that in the DES group.Conclusion:The efficacy and safety of drug eluting stents and drug coated balloons in the treatment of acute myocardial infarction are similar,and DCB may be superior to DES in improving late stage luminal loss.
