螺旋CT在颈椎病患者影像学诊断的效用价值

The Utility Value of Spiral CT in Imaging Diagnosis in Patients with Cervical Spondylosis

目的:探讨螺旋CT在颈椎病患者影像学诊断的效用价值和三维重建后颈椎病不同分型的病理表现。方法:选择2020年5月~2023年2月遵义医科大学第三附属医院门诊、住院颈椎病患者170例为观察对象。其中80例行DR检查,90例行螺旋CT检查,比较两组的检查诊断结果,分析诊断价值,并对螺旋CT诊断明确的患者影像资料进行三维重建进行病理分型。结果:螺旋CT对颈椎病的确诊率(98.88%)显著高于DR对颈椎病的确诊率(86.25%),有统计学差异(P<0.05)。在对螺旋CT诊断明确的89例患者影像资料进行三维重建后得出病理分型为:神经根型38例(42.69%),脊髓型14例(15.73%)椎动脉型11例(11.23%),食管压迫型3例(3.37%),混合型23例(25.84%)。结论:螺旋CT在颈椎病患者影像学诊断中较DR有更高的效用价值,对颈椎病不同分型病理状态有很好的展示能力,为临床医师的诊疗能够提供便捷、全面的指导。

Objective:To explore the utility value of spiral CT in the imaging diagnosis of cervical spondylosis patients and the pathological manifestations of different subtypes of cervical spondylosis after three-dimensional reconstruction.Methods:170 outpatient and inpatient patients with cervical spondylosis in our hospital from May 2020 to February 2023 were selected as the observation subjects.Among them,80 cases underwent DR examination and 90 cases underwent spiral CT examination.The diagnostic results of the two groups were compared,and the diagnostic value was analyzed.Three dimensional reconstruction was performed on the imaging data of patients with clear spiral CT diagnosis for pathological classification.Results:The diagnostic rate of spiral CT for cervical spondylosis(98.88%)was significantly higher than that of DR for cervical spondylosis(86.25%),with statistical differences(P<0.05).After three-dimensional reconstruction of the imaging data of 89 patients diagnosed with spiral CT,the pathological classification was determined as follows:38 cases(42.69%)of nerve root type,14 cases(15.73%)of spinal cord type,11 cases(11.23%)of vertebral artery type,3 cases(3.37%)of esophageal compression type,and 23 cases(25.84%)of mixed type.Conclusion:Spiral CT has higher utility value than DR in the imaging diagnosis of cervical spondylosis patients,and has good display ability for different pathological states of cervical spondylosis,providing convenient and comprehensive guidance for clinical physicians in diagnosis and treatment.
