社区卫生定向服务模式在社区慢性病管理中的应用效果

Application Effect of Community Health Oriented Service Model in Community Chronic Disease Management

目的:探讨社区卫生定向服务模式在社区慢性病管理中的应用效果。方法:选取2020年1月—2023年3月北京市朝阳区东风社区卫生服务中心收治的72例慢性病患者作为研究对象,根据随机数字表法分为观察组和对照组,各36例。对照组采取常规社区服务模式,观察组采取社区卫生定向服务模式,比较两组慢性病知识知晓情况、慢性病管理依从性、生存质量、管理满意度。结果:干预后,观察组慢性病知识总知晓度高于对照组,差异有统计学意义(P=0.006)。观察组慢性病管理总依从率高于对照组,差异有统计学意义(P=0.033)。干预前,两组躯体疾病、精神健康、精力评分比较,差异无统计学意义(P>0.05);干预后,两组躯体疾病、精神健康、精力评分高于干预前,且观察组高于对照组,差异有统计学意义(P<0.05)。观察组慢性病管理总满意度高于对照组,差异有统计学意义(P=0.003)。结论:社区卫生定向服务模式在社区慢性病管理中的应用效果较好,能够提高患者慢性病知识知晓度、慢性病管理依从性以及生存质量,患者管理满意度较高。

Objective:To explore the application effect of community health oriented service model in community chronic disease management.Methods:A total of 72 patients with chronic diseases admitted to Beijing Chaoyang District Dongfeng Community Health Service Center from January 2020 to March 2023 were selected as the study subjects.They were divided into observation group and control group according to random number table method,with 36 cases in each group.The control group adopted the conventional community service model,and the observation group adopted the community health oriented service model.The awareness of chronic disease knowledge,compliance with chronic disease management,quality of life,and management satisfaction were compared between the two groups.Results:The total awareness of chronic disease knowledge in the observation group was higher than that in the control group,and the difference was statistically significant(P=0.006).The total compliance rate with chronic disease management in the observation group was higher than that in the control group,and the difference was statistically significant(P=0.033).Before intervention,there was no significant difference in the scores of physical illness,mental health and vitality between the two groups(P>0.05);After intervention,the scores of physical illness,mental health and vitality in the two groups were higher than those before intervention,and the scores in the observation group were higher than those in the control group,with a statistically significant difference(P<0.05).The total satisfaction with chronic disease management in the observation group was higher than that in the control group,and the difference was statistically significant(P=0.003).Conclusion:The community health oriented service model has a good application effect in the community chronic disease management,and can improve patients'awareness of chronic disease knowledge,compliance with chronic disease management and quality of life,and have high patient satisfaction with management.
