白内障超声乳化吸除术、人工晶状体植入术联合房角分离术治疗急性闭角型青光眼合并白内障的效果分析

Effect of Cataract Phaco,Intraocular Lens Implantation Combined with Angle Separation in the Treatment of Acute Angle-closure Glaucoma Complicated with Cataract

目的:分析白内障超声乳化吸除术(Phaco)、人工晶状体植入术(IOL)联合房角分离术(GSL)治疗急性闭角型青光眼合并白内障的效果。方法:选取2019年11月—2022年10月广西南宁市第二人民医院收治的急性闭角型青光眼合并白内障患者80例(80眼)作为研究对象,随机分为研究组与对照组,各40例(40眼)。对照组采用白内障Phaco联合IOL治疗,研究组在对照组基础上实施GSL治疗。比较两组视力、眼压、前房深度、房角粘连情况。结果:术后7 d、术后1个月、术后3个月,两组视力优于术前,差异有统计学意义(P<0.05)。术后1、3个月,两组眼压低于术前,差异有统计学意义(P<0.05);术后3个月,研究组眼压低于对照组,差异有统计学意义(P<0.05)。研究组术后并发症发生率低于对照组,差异有统计学意义(P=0.025)。术后3个月,两组前房深度大于术前,且研究组大于对照组,两组房角粘连<90°占比高于术前,且研究组高于对照组,差异有统计学意义(P<0.05)。结论:白内障Phaco、IOL联合GSL治疗急性闭角型青光眼合并白内障的效果显著,可改善患者视力,降低眼压,且安全性较高。

Objective:To analyze the effect of cataract Phaco,intraocular lens implantation(IOL)combined with angle separation(GSL)in the treatment of acute angle-closure glaucoma with cataract.Methods:A total of 80 patients(40 eyes)with acute angle�closure glaucoma combined with cataract treated in Guangxi Nanning Second People's Hospital from November 2019 to October 2022 were selected as the study objects and randomly divided into the study group and the control group,with 40 cases(40 eyes)in each group.The control group was treated with cataract Phaco combined with IOL,and the study group was treated with GSL on the basis of the control group.The visual acuity,intraocular pressure,anterior chamber depth and angular adhesion of the two groups were compared.Results:7 d,1 month and 3 months after surgery,the visual acuity of the two groups was better than that before surgery,and the difference was statistically significant(P<0.05);At 1 and 3 months after operation,the IOP of the two groups was lower than that before operation,and the difference was statistically significant(P<0.05);3 months after operation,the IOP of the study group was lower than that of the control group,and the difference was statistically significant(P<0.05).3 months after surgery,the anterior chamber depth in both groups was greater than before surgery,and the study group was greater than the control group,and the proportion of Angle adhesion<90°in both groups was higher than before surgery,and the study group was higher than the control group,the difference was statistically significant(P<0.05).Conclusion:Cataract Phaco and IOL combined with GSL in the treatment of acute angle-closure glaucoma complicated with cataract have significant effects,can improve the visual acuity and IOP of patients,and have high safety.
