抑郁发作患者心率变异性及其与抑郁的关系

Heart rate variability of patients with depressive episode and its relationship with depression

目的探讨抑郁发作患者心率变异性(HRV)的特点及其与抑郁的关系。方法将95例抑郁发作患者设为研究组,99名健康志愿者设为对照组。两组受试者均进行短时程(5 min)心率变异性检测,采用汉密顿抑郁量表17项(HAMD-17)评估研究组受试者抑郁状况。比较两组受试者HRV指标的差异,包括平均正常RR间期标准差(SDNN)、总功率(TP)、低频功率(LF)、高频功率(HF)、平衡指数(LF/HF)。对HAMD-17总分及各因子分与HRV的相关性采用Spearman相关分析。结果研究组受试者SDNN、TP、LF及HF指标均明显低于对照组(P<0.01),LF/HF虽升高但不显著(P>0.05)。研究组受试者HAMD-17的焦虑/躯体化因子分与SDNN、TP、LF均呈负相关(P<0.05),与HF、LF/HF无明显相关性(P>0.05)。结论抑郁发作患者自主神经调节能力下降,HRV可作为辅助识别抑郁发作的生理学指标。

Objective To explore the characteristics of heart rate variability(HRV)in patients with depressive episode and its relationship with depression.Methods Ninety-five patients with depressive episode were assigned to study group and 99 healthy volunteers to control group.Short time(5min)HRVs were detected in both groups and depressive status was assessed using the 17-item Hamilton Depression Rating Scale(HAMD-17).Differences in HRV indices were compared between two groups,including standard deviation of NN intervals(SDNN),total power(TP),low frequency(LF),high frequency(HF),and the LF/HF ratio.Spearman correlation analysis was used to assess the correlations of total and each factor score of the HAMD-17 with the HRV.Results The SDNN,TP,LF and HF index were significantly lower in study than control group(P<0.01),LF/HF ratio increased but not statistically(P>0.05).Scores on the anxiety/somatization of the HAMD were negatively correlated with SDNN,TP and,LF in study group(P<0.05),but weren't significantly with HF and LF/HF(P>0.05).Conclusion The regulation ability of autonomic nerve lowers in patients with depressive episodes and the HRV can be used as an auxiliary physiological indicator for identifying depressive episodes.
