柔性透明材料3D打印在乳腺肿瘤整形外科中的应用研究

摘要 目的探讨柔性透明材料3D打印在乳腺肿瘤整形外科中的临床价值。方法选取2020年1月至2022年2月宜春市第二人民医院收治的行乳腺肿瘤整形保乳术(OBCS)的20例早期乳腺癌患者作为研究对象,收集患者术前MRI和增强CT血管造影(CTA)扫描的原始数据,通过3Dslicer和Mimics软件分别建立乳腺、乳腺癌、血管和毗邻结构3D模型并模拟OBCS手术过程,输出STL格式进行柔性透明树脂材料3D光固化打印,并统计各项指标。结果所有患者均顺利完成手术,平均手术时间为(155.34±20.45)min,平均术中出血量为(115.25±13.09)ml,术前规划符合率为100.0%,整形皮瓣修复成功率为95.0%。术后3个月,患者情感职能(RE)、心理健康(MH)、生命活力(VT)、社会功能(SF)、躯体疼痛(BP)、总体健康(GH)、生理功能(PF)及生理职能(RP)评分均高于术前,差异有统计学意义(P<0.05)。结论通过将柔性透明材料3D打印运用在乳腺肿瘤整形外科中,可以提高整形皮瓣修复成功率,获得较好效果,值得推广。 Objective To explore the clinical value of flexible transparent materials 3D printing in breast tumor plastic surgery.Methods 20 pa-tients with early breast cancer who underwent oncoplastic breast conservation surgery(OBCS)in Second People's Hospital of Yichun City from Jan-uary 2020 to February 2022 were selected as the research subjects.MRI and the original data of enhanced CTA scans,through 3Dslicer and Mimics software to establish 3D models of breast,breast cancer,blood vessels and adjacent structures respectively and virtualize the OBCS surgical process,output STL format for flexible transparent resin material 3D light curing printing,and statistics of various indicators.Results All patients successful-ly completed the operation,the operation time was(155.34±20.45)min,the average intraoperative blood loss was(115.25±13.09)ml,and the aver-age preoperative planning compliance rate was 100.0%,and the plastic flap repair success rate was 95.0%.3 months after operation,the scores of emotional function(RE),mental health(MH),vitality(VT),social function(SF),physical pain(BP),general health(GH),physiological function(PF)and physiological function(RP)were higher than those before operation,and the differences were statistically significant(P<0.05).Conclu-sion By applying 3D printing of flexible and transparent materials in breast tumor plastic surgery,the success rate of plastic flap repair can be im-proved,and better results can be obtained,which is worthy of promotion.
