三维多普勒超声在子痫前期孕妇胎盘血管形成监测中的应用价值

摘要 目的探讨三维多普勒超声在子痫前期孕妇胎盘血管形成监测中的应用价值。方法选取2020年1月至2022年5月抚州市第一人民医院收治的孕11~16+6周子痫前期孕妇30例作为观察组,另选取同期于本院建档产检的30名同孕期健康孕妇作为对照组。所有孕妇均接受三维多普勒超声检查,分析孕妇胎盘血管指数(VI)、血流指数(FI)、血管血流综合指数(VFI)、胎盘体积(PV)和子宫动脉搏动指数(PI)及围生儿不良预后发生率。结果观察组VI、FI、VFI均低于对照组,PV小于对照组,PI高于对照组,差异有统计学意义(P<0.05);观察组羊水过少、胎盘早剥、胎儿生长受限、胎儿窒息、低体质量儿发生率均高于对照组,差异有统计学意义(P<0.05)。结论三维多普勒超声可通过孕妇胎盘VI、FI、VFI、PV和PI等指标评估子痫前期情况,可作为子痫前期首选诊断手段。 Objective To analyze the value of three-dimensional Doppler ultrasound in monitoring placental vascularization in pregnant women with preeclampsia.Methods 30 pregnant women with preeclampsia of 11-16+6 weeks gestation admitted to Fuzhou First People's Hospital from Jan-uary 2020 to May 2022 were selected as the observation group,and 30 healthy pregnant during the same pregnancy who were registered in our hospi-tal during the same period were selected as the control group.All pregnant women underwent three-dimensional Doppler ultrasound,and the vascu-larization index(VI),flow index(FI),vascularization flow index(VFI),placental volume(PV),uterine pulsatility index(PI)and the incidence of perinatal adverse prognosis in perinatal infants were analyzed.Results The VI,FI,VFI in the observation group were lower than those in the control group,PV was smaller than that in the control group,the PI was higher than that in the control group,and the differences were statistically significant(P<0.05);The incidence of oligohydramnios,placental abruption,fetal growth restriction,fetal asphyxia,and low body weight infants in the obser-vation group were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Three-dimensional Doppler ultrasound can evaluate the condition of preeclampsia by measuring indicators such as VI,FI,VFI,PV,and PI in the placenta of pregnant women,and can be the preferred diagnostic method for preeclampsia.
